Opening to the patients and educating in preventative care can be the key to some of the positive changes that our community needs. Since medicine is an evolving profession healthcare providers should keep up with the most up to date information on how to care for their patients. Through this course I have learned that history taking and performing a physical assessment and being able to differentiate normal from abnormal findings is one of the most important roles of a health care provider. If an accurate physical assessment is not performed, whether for baseline data or when the patient’s condition changes, then the patient is not receiving the level of competent care he deserves. Family Nurse Practitioners are expected to safely provide and meet the healthcare needs of people across the lifespan and diverse cultures through evidence-based care. According to National Governors Association (NAG) (2012), the growth of the aging population combined with the initiative of the Affordable Care Act, the problem of primary care providers shortage is worsening. Nurse Practitioners (NPs) are educated and prepared to perform primary care services as physicians do. More importantly, studies showed that NPs’ role as primary care providers achieve equal or higher patient satisfaction rates (Evangelista et al., 2011, Albers-Heitner et al., 2013).
Advanced practice nurses (APN) have a vital role in the future of health care, especially since the enactment of the Affordable Health Care Act. With more citizens having health insurance coverage they will be seeking health care providers, and there are not enough physicians to care for them all. According to Letiziam (2014), advance practice nurses are licensed autonomous health care providers that have been trained to evaluate, diagnose, and treat patients and their conditions. Advanced practice nursing is an umbrella that covers four separate roles of nurses, this includes: the certified nurse midwife (CNM), certified registered nurse anesthetist (CRNA), certified nurse practitioner (CNP), and certified nurse specialist (CNS).

Advanced practice nursing (APN) can be defined in many ways and differs from basic nursing practice. ANA (2010) defines APN as incorporation and application of evidenced based practice that is accomplished during graduate school. The AACN distinguishes the role of the APN by stating that they have a direct clinical focus. AACN states that interventions taken by APNs have an impact on patient care outcomes. APNs must put patient care and health care policy first.
After 10 years working as a registered nurse in many aspects of healthcare, felt an overwhelming desire to advance my career and myself to expand my role in caring for others. Becoming a nurse practitioner has always been a goal of mine. During my time working as a nursing manager, I became acutely aware of the need of skilled and talented providers that desire to provide passionate care to all individuals. I became especially concerned with the needs those with limited access to healthcare services. I knew my desire to care for individuals in a more autonomous role and I set out to develop the skills needed to meet the need I witnessed.
The demand for highly skilled nurses and physician shortage prompted the introduction of Advanced Practice Nurses (APNs) in the delivery of primary care in the United States. The increase in education, training, and enhanced professional standards allowed the nurses to be clinically capable. As a result, it transformed APNs in advanced roles which were only performed by physicians in the past. The increased visibility of the APN role provides a “historical benchmark” that allowed for other countries emulate this success (Schober, 2016).
Coaching is defined as “a purposeful, complex, dynamic, collaborative, and holistic interpersonal process aimed at supporting and facilitating patients and families through health-related experiences and transitions to achieve health-related goals, mutually determined, whenever possible.” 1 It serves as an intervention implemented and mediated by the APN using self-reflection, interpersonal, clinical, and technical skills.1 Understanding patients’ experiences of transitions, defined as “passage(s) from one life phase, condition, or status to another” 2 is imperative for the APN to successfully coach. My knowledge in the Doctor of Nursing Practice (DNP) continues to expand as I progress through the DNP program. The knowledge I have attained while in the study of the DNP I and DNP II is priceless. For example, recognizing stakeholders and utilizing networking has helped me to progress on my DNP project (Gokenbach, 2012). I also have a better understanding as well as the purpose of a DNP project. My mentors have been great in directing me in remaining on the right track.

Reflecting on the past seven weeks I have acquired countless knowledge, which I will use to further strengthen my profession as an Advanced Practice Nurse. The course allowed me to think beyond my current practice knowledge and acquired innovative ways to evaluate the situation at hand. The learning objective in program outcome four helps set standards that I will use to guide my clinical practice to meet various healthcare needs. Using the case studies has helped to further enhance my knowledge on disease physiological state, using differential diagnosis, disease manifestations, and clinical presentation. It has also taught me the skills on how to differentiate between similar diagnoses to properly identify the problem and treat the patients.
As there continues to be an initiative to improve quality healthcare while simultaneously reducing costs, the importance of the nurse with advanced education in transforming healthcare delivery and healthcare policy will continue to grow.
