On March 15, 2016 at 4:34 am, I was notified by Elisha Smalls that she had discovered red spots on her arms and mid back and had an itch. The employee was instructed to leave the facility and see her physician as soon as possible and to follow-up regarding the visit. At 11:14am, the employee called back and stated she had chicken pox and was in contact a resident in ALF that had shingles. It was discovered that the employee di d work with the resident on 02/24/2016 and 02/25/2016.It was discovered on February 24, 2016; a resident in Assisted Living did exhibited signs and symptoms of possible shingles. The resident was seen by the Nurse Practitioner, Jody Eblen for a rash with pain, the resident was started treatment with antibiotics on

On April 12, 2016 I was assigned case 16-125169 which involves Ms. Mya Navia a fourteen year old Francis Hammond School Student. On April 13, 2016, I was assigned a second case which was 16-125698 involving the same juvenile. On April 13, 2016 I touched base with Arrosa Kanwai (mother) who had previously denied me access to her daughter (16-119375).

Introduction Alzheimer’s is a chronic neurodegenerative disease that usually starts slowly and gets worse over time. The most common early symptom is difficulty in remembering recent events or short memory loss. As the disease advances, symptoms can include difficulty with language, disorientation, mood swing and behavior problems. As a person 's condition progressed, they often withdraw from friends and family. Slowly, bodily functions deteriorated and eventually death occurs.
WOOSTER — The Wayne County commissioners and Job & Family Services workers wore purple Wednesday to raise awareness about elder abuse. Director Rich Owens and Melissa Plotkin, who investigates complaints of elder abuse, talked about how the number of cases have been on the rise, how abuse/neglect/exploitation cases are investigated and the confidentiality of each investigation. Over the past two years, Plotkin has been investigating between 280-290 cases annually. Which is up over previous years.

While conducting a face to face interview with Marilyn Harrison the director of the operation, Marilyn stated that Maleigh B. the victim was wearing flip flops on the day of the incident and was informed of the incident when Thelma Honea had brought Mayleigh to her after she fell on the playground, Ms. Harrison stated the child was crying and saying her arm was hurting but there was no swelling of the arm and Mayleigh was moving it around and using it.
