The identified selected funding agency that has been identified is the Harry and Jeanette Weinberg Foundation (http://hjweinbergfoundation.org/program-areas/older-adults/) (http://hjweinbergfoundation.org/grants/). I modified the grant as a result of search because this foundation provides money for seniors and veterans. This grant proposal will be written to support two identified vulnerable populations the Foundation has determined as underserved. This will make this grant stand out to the funders and evidence its uniqueness by demonstrating the goals and objectives outlined by the Foundation, inclusive of two underserved populations.
This program is a non-medical home care program that aids low-income seniors, adults and children with disabilities. This service pays for someone to go out to the client’s home and help with “chores.” There is also the Area Agency on Aging (AAA) that helps agencies plan, develop, and coordinate services for older adults 60 years of age or older. The AAA gets about 2 million dollars in grants and gives those funds to local non-profit agencies. The AAA funds more than a handful of services including; outreach, medals, legal services, family caregiver support and many more.

When a senor requires additional care, there are various decisions that need to be made. These include whether the senior wants to remain in their current home, the ability to handle an ongoing medical condition while at home and the financial capacity to invest in professional care services. Long-term senior care plans will be considered according to the individual’s mental and physical capabilities as well as their social needs. Finding a Care Provider After the decision to hire a care service is made, the next important step is to find the right care provider on a short or long-term basis.
Reading this article for the elderly care, I feel that ethical issues commonly occur anywhere in the treatment of older patients. I had a clinical experience both in an acute-care hospital and in a long-term care facility. Before working in a long-term facility, I was not aware of how many ethical principles were violated in the treatment of older patients as a daily routine as stated in the article. In reality, there are many situations in which older patients don’t completely exhibit their autonomy because they are vulnerable physically and emotionally and dependent on others. Therefore, they become more conscious of caregivers or healthcare professionals.
3.1 Explain the theories that underpin health and social care practice The main purpose of dementia care worker is to provide support for people with dementia and helping them to improve their sense of well-being, to maintain their independence and to put them in more control of their lives. It is underpinned by values of respect for persons, individual right to self-determination, mutual respect and understanding. It is enabled by cultures of empowerment that foster continuous approaches to practice development.’ This type of care approach is focused solely on the person and the concept of personhood (HSE, 2010). It is imperative that the nurse hears the voice of the older person.
This is more than just the financial resources that they have, but also their psychological and physical health and their spiritual outlook on life and the situation. One important factor is the caregiver's location and place of residence in relation to that of the person to be cared for. It must also be remembered that the carer has a life outside of this role and, therefore, important factors in caring include other day-to-day roles such as being an employee, a parent and a professional person. The carer is also affected by the opinions and demands of people outside the caregiving relationship. Caregivers are also facing a health care system that seems to be placing more responsibilities on caregivers while providing less and less
Some elderly people can survive and perform their daily routine on their own while others cannot due to their age or their illness. Those who fall into the latter category would require extra care at home. This home care isn’t covered and if they can’t afford it they won’t have access to it which will result in lower quality of life. (Simon, 2013) The solution to the issues

Tom Kitwood (1997) cited in (The Open University, 2017) supports the approach of seeing and treating people as individuals, he calls this ‘person-centred care’. This approach looks at the physical, social and psychological needs of the individual. Person-centred care encourages people to have more involvement in making decisions about their care so they get the support and service they need. There are three main types of long-term care settings such as residential care homes which offer different degrees of personal care, Nursing homes offer care which requires the skills of qualified nursing staff and long-stay hospitals which offer a more specialised medical care. (The Open University, 2017).
Health care practitioners most see things from the older person’s perspectives by showing compassion when delivering care to the patient along side emotional support
The fifth theme involved the loss of freedom. The cognitive capacity in this study refer to the caregiver’s information processing ability. Structure Providers: Institutional care quality was the single theme in this category. Appraisals were mostly negative and included feelings of resentment, a lack of support from family members, financial strain, and loss of freedom. Coping strategies included support groups, caring for one’s self, religion, and humor.
