Clinical Pharmacist involvement in hospitalized patient care processes has showed improvements in better efficacy and medication safety by reducing number medication errors and adverse drug events .This has an impact on decreasing health care costs.

As a drug expert, main responsibilities of clinical pharmacists include improving appropriate prescribing and administration of drugs, monitoring patient adherence to therapy, providing drug information consultation to providers, monitoring patient responses and laboratory values, and providing patient and provider education.

Cesarz, et al showed that improvement of patient care associated with suboptimal prescriptions with pharmacist review of discharge prescriptions.6 Weant et al reported a great impact of ED pharmacist on optimizing empiric antibiotic selection and dosing in sepsis management in ED7. Systematic literature review was conducted by Cohen showed that Pharmacist has involved in providing consultations on medication issues, identifying and reducing medication errors, and conducting medication histories at hospital admission. Each Clinical pharmacist is assigned to certain area of the ED .These areas include:

• Urgent areas: where patients with moderate conditions are seen
• Intensive care area: where patients with moderate conditions are seen
• Short stay areas: where patients with acute conditions requiring hospital stay from 1-3 days are seen

As part of improving the service in ED, we decided to conduct Pharmacist led Pharmacy round (PLPR) led by Pharmacist to ensure best patient care is provided by pharmacist, provide educational at point of care and address any pharmacy related issues.
