Once the plan is developed, it should be implemented effectively and monitored as per need to identify the effectiveness of therapy and evaluate the outcomes. If required, the plan should be modified and monitored again until the patient achieves optimal healthcare outcomes.
Question 2 : Are documents like this important to our profession? Why or why not?
Answer : Yes, I think that documents like these are very important to our profession. As we know, the profession of Pharmacy is constantly evolving and demands a Pharmacist who can work as a team with other healthcare professionals to provide optimal patient outcomes. In such a scenario, it becomes necessary to have a guiding source where Pharmacists can look for to understand the need of today’s market and provide patient care as per the need of the time.
Hence, I think that these documents can guide Pharmacist to provide better patient care and help the patients reach optimum healthcare outcomes and enjoy an improved quality of life.
