Interdisciplinary Team Role In Health Care Essay

The interdisciplinary team is composed of many health care providers and each has a role in promoting patient adherence. One study states, “This intervention consisted of a patient brochure and 2 group education sessions (2 hours each) with 1 session delivered by a physician and the other by a pharmacist. In addition, 1 or 2 individual sessions (40 minutes) were supplied by a nurse” (McDonald, Garg, & Haynes, 2002). This study, in particular, exemplifies the roles of three different team members and their roles in improving patient adherence. Nurses attempt to improve patient adherence through a few different methods. For example, bi-weekly automated telephone assessment and self-care education call with nurse follow-ups. Those who went reported fewer problems with medication adherence (McDonald & et. al., 2002). Pharmacists also attempt to improve patient adherence through 4-week follow-ups. Patients who went to the 4-week follow-up had higher adherence rates (Clifford, Barber, Elliot, Hartley & Horne, 2006). By creating a relationship with the patients, the pharmacist is able to meet the patient’s needs and give detailed advice on the medications. Lastly, the prescribing provider used different techniques such as telephone prompting, patient reminder letters, orientation statements, clinic organization, and advanced planning of future appointments. The physician also created a therapeutic relationship and described in depth the information regarding the medication (McDonald & et. al., 2002).
