Family: Jehmari resides with Doxie Samuels (mother) and Junior Copeland (father) in a 2 family house in Plainfield. Jehmari has a close relationship with his mother. He also have a close relationship with his older sister who does not resides in the family home. CM asks Mrs. Samuels how Jehmari is behaving in the home. Mrs. Samuels reports that this time Jehmari is behavior has improved. She reports that he has been doing better with managing his anger outburst in the home. Jehmari reports that he promise his mother that he going to continue working on better controlling his anger for when he becomes upset. Jehmari states that his mother is very supportive of him and he knows that she only wants the best for him. The family is in a good space at this time. CM will be looking to transition youth out of CMO level of care the beginning of July.
Action: At this time the family is in a good space. Mrs. Samuels will continue to be supportive to Jehmari needs. …show more content…

Rubina Sharief with F25.0-Schizoaffective Disorder, bipolar type. He is compliant with taking his medication at this time. Jehmari continues to receive therapeutic supports (i.e. medication management and counseling services) at NuView Academy Annex. He is currently taking Lithium Carbonate 300 mg 1 cap 2x daily; Lithium Carbonate ER 450mg 1 cap @ 8 pm and Quetiapine (Seroquel) 100 mg 1cap 2x daily. Youth medication is prescribed by Dr. Zuver, school psychiatrist. At this time Jehmari is complaint with taking his medication daily. There was no behavioral incidents reported at the time of
