Heart failure readmissions for healthcare agencies continues to be an area of concern due to the cost associated with each readmission. Readmissions to the hospital for heart failure is tied to reimbursement and financial penalties. Developing a plan to combat readmission is a difficult task.
Staples & Earle (2008) used a phenomenology research design, where they used a convenience sample of CHF patients to determine effective technologies for monitoring patients with heart failure admissions and mortality. The effectiveness of CHF patients through the use of telephonic assessments and interventions was implemented. Congestive Heart Failure study participants (n=591) were managed by a team of registered nurses and nurse practitioners. Data was collected using a telephone log and appropriate medical protocols were provided. Data was analyzed; determining frequency of calls, level of care required and scope of practice needed to ensure proper care of the patients.

The United States is composed of many cultures, traditions and religious beliefs, sometimes making it difficult to find the right treatment for each individual patient. Some may believe their religion will cure them from any disease and therefore reject any medical care, which can lead to unplanned visits to the emergency department, and to add to the situation there is also language barriers. “Members of certain religious communities are convinced that their religious beliefs and practices reduce their risk for chronic diseases (Christiansen et al). Religious beliefs can influence members to maintain a healthy diet and lifestyle, which will decrease their risk for heart failure.
Congested heart failure (CHF) is one of the leading causes of death in the United States (Centers for Disease Control and Prevention, 2015). There have been research studies conducted to provide evidence-based practice in decreasing the effects of CHF. There is evidence that links exercise, diet, and education as key factors that decreases mortality and morbidity in patients with CHF. All healthcare settings can utilize and implement evidence-based practice when caring for patients with CHF. Nurses should promote education about CHF and the benefits of regular exercise and diet (Smart, 2010).
The target for cholesterol is less than 300 mg, and I achieved that with an average of 126 mg. For dietary fiber, the target level is 25 g, and I only got an average of 13 g. My average distribution of calories over the two days was not very close to the target values. The target range for the percent of calories that carbohydrates contributes is 45-65% calories while I got only 35% calories. For protein, the target is 10-35% of calories. I got 17% calories from protein, so I was within the target zone for calories from protein.
