This case is about a forty five year old man named Colin. Colin has a five year-old son named Toby who lives with his ex-partner Patricia and her boyfriend Jon. Colin is desperately seeking work after losing his job through redundancy. As a result, Colin signed on for benefits and due to being out of work for 12 months was eligible for the government work programme. He left the programme resulting in losing his entitlement for his job seekers allowance and housing benefits.
Circumstance: Ms. Smalls (MHP), Mrs. Gailliard (MHS), Clarence and Ms. Elizabeth Strong (DSS Worker) schedule medical appointment with the MUSC Foster Care Clinic. Action: MHP called Tara Peevy, RN at the MUSC Foster Care Clinic after MHS explained leaving several messages. Ms. Strong explain emailing the referral form to the clinic. Machelle Green explain receieing the referral form, however unable to reach the DSS worker for additional infromaiton.
Palmer Community Center was once and still is a beacon structure that encompasses the history of Rockbridge County, Virginia. Since opening its door as a school house, to its transformation to a community center, Palmer is a structure that is currently used by its alumni and community members. This iconic building set in the Buffalo Community should be protected for future generations to enjoy. Established in 1902 Palmer served as a school house, referred to as Palmer Academy until 1906, and then evolved into Palmer High School from 1906-1920. From 1920-1956 it had become known as Palmer Graded School, however, in 1956, Palmer School fell vacant and was no longer used for education.

Successful CC =1 CM received a call from Jennifer Wisely (DCP&P worker) for Alexia (youth). CM discussed her concerns with DCP&P. CM was informed that DCP&P met with Ms. Marshall (caregiver) and DCP&P is closing youth’s case before Friday, 3/17/17. DCP&P stated that they were open with the family due to child welfare; DCP&P reported no neglect from family. CM informed DCP&P that CM is unable to reach caregiver and needs to schedule a meeting for this month. DCP&P noted she will contact caregiver and have her call CM regarding this matter.
Background statement: Heritage Valley Medical Center has had a wonderful reputation for providing excellent health care services to their community. Initially, their community was 80% Caucasian, 40% African American, and 5% Hispanic. However, in the last 5 years, the population has changed to more minorities and the whites have moved out to the suburbs. This caused the Center’s occupancy rate to go down 40% because many of their traditional, more affluent, private-pay patients had left the neighborhood. To bring in revenue, they campaigned to bring in more Medicaid patients.

This is due to the poorest living in run-down areas with no reasonably priced nutritional food available from local shops. If they are unable to afford the costs of travelling to areas supplying this, or if they are time poor then cooking beneficial meals is difficult then they are more likely to buy cheap, fatty products. The involvement of the government in the population’s lifestyle means that they should be accountable for the effects that come alongside an unhealthy regime, such as an increase in health problems such as Coronary heart disease. This means that the treatment an individual receives shouldn’t be approached differently
As more people seek for support to get through difficult times, the demand of human services agencies has been increasing. Without agencies and organizations, the goals of the human services field would be impossible to satisfy. The purpose of human services is to improve the overall quality life, both individually and collectively. Just as there are many causes within the human services field, so too are there a huge numbers of organizations whose target is to accomplish these goals. In our visit to one local community based agency, we investigated the mission and structure of this human service organization, requirements of the agency’s services, job titles and responsibilities and how professional ethics are applied in the agency.

Life at the centre could have been worst after staff tried to install a 25 lb tank on the compound. It resulted in a female cook having one of eyebrows burnt off when the flames flared up from the stove.
