For this assessment, I have chosen Delaire Nursing & Rehabilitation Center located in Linden, New Jersey. I was excited about this project because of the emotional attachment that I have to Delaire. My grandfather had been a patient at Delaire up until his recent death in October. My family had been in charge of his primary care until his diagnosis of dementia, that later was identified as Alzheimer’s disease, became unmanageable due to their limited training and education in dealing with someone with AD. The individual units include: assisted living care community, sub acute rehabilitation care, long-term care, and memory care. When you walk up to the facility from the outside, there isn’t much to look at. The building is standard, gray, and looks like any regular building. The real beauty is once you walk through the door. The décor is very naturalistic with wooden bookshelves, mahogany lounge chairs in the conference areas and meeting rooms, various paintings including some of the residents work, and an overall look of cleanliness. The hallways were well lit and provided ample space for wheelchair accessibility. There was carpet in all common areas, nurses’ station, and bedrooms; however, it was pleasing to know that the rehabilitation clinics were not carpeted. Upon entering, the help desk is the first place you interact with an employee. The worker was very polite and welcoming when I visited and was always willing to answer my questions and concerns. There were flyers and information about the facility, upcoming events, outside activities, etc… that kept everyone informed and up to date. Everything seemed functional for impaired older persons and the public. There were televisions available for the residents along with tables to play bingo, card games, and material to engage in arts and crafts in the common …show more content…
The first unit to be covered is the assisted living care community. There are 40 beds available for the residents. There are calendars set up advertising activities in the facility and in the neighborhood. They can engage in community outings, religious programs, and walk groups to name a few. They are free to engage in past activities that interested them and they are able to share them with the other elders (i.e. knitting, playing an instrument, bingo, cards, painting). The second unit is a 60-bed dementia unit that specifically cares for those with some form of dementia. Their activities include visits from volunteers, churches and schools, musical programs, ministry work, and pet therapy. The unit offers 24 hrs a day Nursing and Certified Nursing Care that create a plan of treatment individually tailored to the patients through coordination between the family, physicians, and rehabilitation services team members (ST/PT/OT).
