Besides just not being able to administer help to a vast majority of addicted people, nearly the totality of patients, substance abuse treatment in psychiatric institutions has historically been unhelpful for those who receive it. This unhelpful nature, once again, is due to the fact that patients do not trust those who care for them, as explained. However, this also changes when care institutions adopt organized care policies, as many of these institutions did see tangible and beneficial change to those incarcerated. This is due to the fact that the care was removed from the concept of policing and jailing, and instead sought to pair the concept of care with rehabilitation and community (Levenson & Samra, 2023). During the 1970s and 1980s, the use of illegal drugs was growing; which undertook a war on drugs. As of June 2001, there were a total of 697 drug court programs, serving around 226,000 offenders and another 427 programs being planned (Office of Justice Programs, 2001). The drug court can be seen as a social movement to crack down on drugs. Although the drug court model continues to evolve, there are some key components. Some of these key components are, a non adversarial approach that emphasizes teamwork; eligible participants are defined early and promptly placed in the drug court program; and abstinence is monitored by frequent alcohol and drug testing, and so on.
Society most of time tends to be keen on helping each other. One way we help each other is by allowing inmates, no matter the crime, to join rehab. Steve Earle the author of ‘A Death in Texas’ was in drug rehab at one-point, finished rehab, and got clean of drugs. Earle then wrote about Jonathan Wayne Nobles a man on death row for killing two people. While Nobles was on death row he took drug rehab and got clean of his drug addiction.

The idea behind these program was to help treat the offenders for their substance abuse disorders while still holding them accountable for the crime that they had committed (Lutze & Wormer, 2013). Many studies have been conducted in order to assess the effectiveness of drug court programs across the country. In a qualitative study done by Gallagher 100 participants of the drug court program were examined. This study found that of the drug court participants, seventy-nine percent were not rearrested in the follow-up period. Twenty-one percent of those participants were rearrested (Gallagher, 2014).

Harm reduction is a pragmatic approach to reducing individual and social harms associated with drug use, especially the risk of HIV infection. This approach accepts that certain interventions focused on diminishing the harmfulness of a substance, even if they increase the extent of substance use, may be able to reduce the total adverse consequences on the individual, as well as society (Meulen & Ka Hon Chu, 2015). In regards to high risk behaviour for the transmission of HIV in prison (e.g., needle-sharing), harm reduction approaches acknowledge that there are no known effective solutions for completely eliminating drug-use or drug-related problems in the community
