Circumstance: Ms. Smalls (MHP), Mrs. Gailliard (MHS), Clarence and Ms. Elizabeth Strong (DSS Worker) schedule medical appointment with the MUSC Foster Care Clinic. Action: MHP called Tara Peevy, RN at the MUSC Foster Care Clinic after MHS explained leaving several messages. Ms. Strong explain emailing the referral form to the clinic. Machelle Green explain receieing the referral form, however unable to reach the DSS worker for additional infromaiton.
Received a report on 10/7/15 stating Ms. McGinnis was not given medication properly and scoring a 0 on MMSE. Son, Michael McGinnis reported he gave her dose of Methadone because she was hurting. Ms. McGinnis admitted to WBMC on 10/6 with chest pain and hypertension. While on medical unit, she was confused, pulling the cardiac monitor off, yelling, screaming, cursing the nurses, disrobing, and refused meds. On 10/7 she transferred to Senior Care she remained until 10/28 3:30.

Michaelin Higgins-Wiggins was a clinical assistant in Sutter’s Shared Services Department. She reported to her physician that she was experiencing stress as a result of her involvement with both her direct supervisor and the human resources department on the job.
Dana Wong is a seventy-year old female who had an ischemic stroke, and is entering our care facility after a seven-day stay at Mercy North Medical Center. The stroke has left her in a wheel chair with decreased mobility on the left side, slurred speech, and relaxed features. The referral came from Dr. Parker on September 12, 2015, which was four days after her admission in the hospital, and request that Mrs. Wong undergo two months of physical, occupational, and speech therapy before she returns to her home at Green Meadows, an assisted care living facility. Dr. Parker noted that Mrs. Wong can benefit from our services, and believes that she can make an adequate recovery, because she is determined to maintain her independent lifestyle. Mrs. Wong has full coverage from Blue Cross Blue Shield, who has agreed to pay for her stay at the facility for a maximum of two months.

He reports after his 16 hour trip, he went to a mechanic shop to call 911 for chest pain and suicidal ideation without a plan after stressing about having a place to stake for a few days. SC completed home visit with Pa and DCW on 01/22/2016. Pa appeared properly groomed and dressed in white T-shit and navy blue sport pants with white strip on the sides and white cotton soaks and no shoes on his feet. Pa was sitting in a pink leather reclining chair with a severing tray in front of him waiting for his breakfast while watching the news. All utilities appeared to all be in working order. Pa lives in the basement of the DCW split level rented town home.
One of her clients on her case load is a 60 year old, bed bound, diabetic person. This client does have provider staff, but they only come in 4 hours a day (2 in the morning and 2 in the afternoon). The provider leaves at about 7pm, so this client is in her bed until the next morning when staff arrives at 10 or 11. This client has spent 12 hours in her bed, with possibly soil linens. The client does not want to be placed in a nursing home, does not want more provider hours, and does not want move with family.

Using technology can be good and bad in many ways for instance if you need a ride home you can call or text your parents/friends for one. I think technology is bad even if I use it it will always be bad for us it. I use technology when i 'm bored, stressed, or sad, it helps me relax in many ways. As I progress in my career, I feel that it is important to develop a personal style and technique that clients can relate to. I am also interested in learning more about assessments and evaluations as they drive the rehabilitation plan. While I have assessed hundreds of clients, I realize that there is more to learn. For instance, the trend over the next five years will be an increase in applicants who receive state-plan home and community based services. The Final Rule issued by The Center for Medicare and Medicaid will require that individuals receive services in the most integrated settings.
