Based on the current needs assessment, the vision of the Tarrant County Homeless Youth Program (TCHYP) is to significantly minimize runaway youth – individuals less than 18 years of age, and street youth – less than 21 years of age, who are experiencing homelessness throughout Tarrant County. In order to carry this out, our mission statement is as a nonprofit organization dedicated to assisting the homeless youth in Tarrant County to become self-sufficient through safe shelter or stable housing, case management, wrap around services, and regular status follow-ups. Moreover, although there are several agencies utilizing shelters in the area, our program’s main focus will be to ensure that effective interventions are in place to build skills and capacities that contribute to a healthy, optimistic, and productive functioning of children and youth into adulthood. Additionally, an individualized biopsychosocial assessments will be performed followed by an intensive case management plan tailored to each individual, based on their needs and strengths. …show more content…

With that in mind, outreach workers will be also encouraging the homeless individuals to become part of the (TCHYP), where additional case management will be offered/provided to the clients with the plan of becoming and remaining self-reliant.
