I spoke with the patient 's wife via phone, The wife states that she needs additional assistance for the patient. She states that she is aware that the patient 's health is declining and that she wants him to be comfortable at home. I discussed hospice services with the patient and the wife. The wife states that she would like hospice to evaluate the patient, the patient also agree for hospice to evaluate. Case reviewed with the patient 's nurse, the charge nurse, case management and hospice.
The reporting party (RP) stated on 8/1/15 as she was assisting a Memorial Hospice patient shower she heard the front door alarm sound. The RP stated a staff person (Abundio Serenio) was assisting with the shower at the time of the incident. The RP stated she heard the sound of running footsteps and a woman 's voice screaming she wanted to leave, she had to get out of here. The RP stated she heard the staff call out the name "Claire" during the incident. The RP stated she finished assisting with the shower and went to the hallway and observed "Claire" running towards her saying "Chris" kicked her.

As an immigrant, I was quite hesitant about accepting new challenges. The fear of others’ judgment regarding my performance hindered my progress. However, through serving others in various capacities, I was able to interact and help people in my community, who themselves helped me overcome my fears. Hospice and palliative care can be easily intertwined; they are both concerned with promoting comfort and relieving patient pain. Hospice and palliative care, however, are different in some aspects. Patients who receive hospice care are nearing the end of their lives and there is no effort to cure their disease; the goal is to provide pain relief, a sense of belonging from family and friends if desired, support through the dying stages, and to assure that the person is able to die with dignity. Palliative care is also focused on reducing discomfort; however, the patient receiving care can be at any stage in their disease. Additionally, palliative care can also be administered during a time when a patient is receiving treatment to cure their illness.
My Intent to Be a Nurse Practitioner Coming from a third world country where there aren’t many opportunities for work and funding for education, proceding to nursing school was a grand opportunity. The privilege to attend a government subsidized school where top students in the region compete to get into the program that allowed only 60 students per year was indeed a blessing. An event that was significant to me throughout clinical placement was when I forgot to introduce myself to a visually impaired client.
