Determining nurse-to-patient ratios in nursing facilities remains a challenge for the nursing profession. There are many factors to consider when determining staffing methods, such as cost, nurses’ satisfaction, patient outcomes and safety. Mandating ratios is one attempt at ensuring nurses’ workloads do not exceed what is needed for adequate patient care and safety.

However, poor patient outcomes not only result from short-staffed nurses, but can also result from inadequate nursing assistants as well. NAs play a great role in providing basic daily care of patients. These professionals are very crucial in the healthcare industry but sadly, there are NA shortages.
Due to hospital care reaching an all-time high in America, we need nurses now more than ever before. Currently in America, we have an issue with nurses having too many paperwork to fill out. In the article “We Need More Nurses” by Alexandra Robbins argues we need more nurses in the hospital. Nursing shortage has been a common issue throughout the world. Because of this issue others are being affected in many different ways.
A report by the Agency for Healthcare and Quality in America projected that hospitals involving increased levels of Registered Nurse (RN) staffing showed decreased degrees of substandard patient outcomes and increased nurse staffing ratios were linked with a 2% to 25% decrease in substandard outcomes (Mark & Stanton, 2010). As a result, education on nurse staffing and the influence on quality of patient care warrants evidence based decision on existing and forthcoming practices. The influence of nurse staffing ratios on quality of care has been observed in four studies. Hospitals using decreased levels of nursing staff faced a 7% escalation in 30-day mortality and a 7% escalation in failure-to-rescue.
In order for hospital units to maintain somewhat safe ratios a sliding scale approach formula is used by a lot of units across America. An article from The Journal of Nursing Administration on Nurse Labor Data: The Collection and Interpretation of Nurse-To-Patient Ratios states that “almost all units used a sliding scale approach to account for census changes” (Minnick & Mion, 2009, para. 23). If the minimum ratio is four patients to one nurse for said unit the admission of more patients beyond the minimum ratio requires that an additional RN is needed in order to rebalance the ratios to meet 4:1 (Minnick & Mion, 2009). The example sliding scale formula mentioned above may not always work as favorably as nurse managers would like it to. The more patients with high acuities are admitted the more caseloads the nurses have to take on, more so if the unit is short staffed and there are no extra nurses that can be called in to lessen the workload.
Short staffing is one of the many challenges nurses encounter in the work environment. The impacts can be detrimental primarily to the patient’s outcome. To examine the effects of short staffing, research was conducted on 36,539 hospital inpatients to evaluate the amount of those exposed to an understaffed shift and how many patient outcomes resulted in a NSO (Twigg, Gelder, & Myers, 2015). NSO’s are nurse sensitive outcomes based on the nursing care provided to the patient. Patients exposed to short staffing had an increase of greater than one chance of NSO’s compared to patients not exposed (Twigg et al., 2015).
Improvement of nurse staffing levels will improve the quality of care our patients receive. Memorial Hospital should adapt a staffing grid for each unit. According to the guidelines followed by California there should be a minimum number of patients per nurse. This number will vary
I am deciding to apply to the University of Wisconsin-Madison because I am intrigued by the opportunities the university has to offer. During my time at UW-Madison I hope to participate in intramural sports, get a job, receive an excellent education, and much more. The city life offers a diverse culture and allows you to learn about and to appreciate life. As I want to be a part of the nursing program I would utilize the opportunity to work at the University of Wisconsin Hospital and to work alongside other students trying to achieve the same goal as I am.

A real shortage is circumstances where experienced people are not available for a certain vacancies due to some reasons (Wildschut&Mqolozana, 2008). A news article written by Salma Khalikin in Straits Times stated that according to current situation Singapore may not be able to create enough nurses for upcoming years. The impact of nurses shortage may causes increase work load for nurses which subsequently may increase the risk for nurses made errors in clinical, the risk of increase hospital acquired infections which cause by viral, bacterial, and fungal pathogens. More over the impact of shortage of nurses may also increase death rate, and also increase the risk of occupation injuries and exposure in working environments.
Nurses play an essential role in the healthcare industry. The nurse workforce is made up of licensed nurses: registered nurses (RNs), licensed vocational nurses (LVNs) and licensed practical nurses (LPNs), along with nurse aides. Registered nurses are responsible for assessments of patients’ needs, development of care plans, medication administration, and treatments, while licensed vocational nurses perform specific care under the delegation of the registered nurses and supervisions. Understaffing in hospitals is a major problem that has been affecting healthcare workers and patients for many years. I have chosen to highlight understaffing as a patient safety issue because of the consequences that can arise from it. Shortages in staffing can result in an increase of infection rates (Stone et al., 2007) leading to complications and poorer patient outcomes (Needleman et al. 2002). This particular issue is of interest to me because I have experienced and witnessed it myself.
