Introduction Ms. Jones is a 28 years old African-American female, who came in to Shadow General Clinic for a pre-employment physical. The patient is the main source of information for obtaining her history and physical. Ms. Jones was last hospitalized in Shadow General Hospital six months ago for a wound on the bottom of her right foot. Her past medical history includes asthma, which was diagnosed when she was 2½ years old, type 2 diabetes diagnosed at age 24, hypertension and PCOS (polycystic ovarian syndrome) four months ago during her gynecologist appointment. She was last hospitalized for asthma when she was 16. Her hypertension is being controlled by diet and exercise. He had hypertension, high cholesterol and diabetes. Mother is 50 and has hypertension and high cholesterol. Her brother Michael age 27 is obese and younger sister Britney, 15 has asthma. Paternal Grandfather died at age 65 from colon cancer, he also had hypertension, high cholesterol and diabetes. Paternal grandmother is 82 and has hypertension and high cholesterol. Maternal grandfather died at the age of 80 from a heart attack, he had hypertension and high cholesterol. Maternal grandmother died at age 73 due to a stroke and had hypertension and high cholesterol. Paternal uncle has history of alcoholism. Ms. Jones is in a new relationship and lives with her mother and sister but is planning to move out in a month once she has started her new job. She very involved with her church and has a strong connection with her faith. She is not sexually active and is using birth control pills. She does not smoke but reports that she used smoke marijuana from age 15-21. She drinks alcohol, socially with friends every couple of months. Never used street drugs. This condition is link with patients who are obese, is diabetic and has polycystic ovarian syndrome (Sander, 2016). This condition is all present with Ms. Jones. Another problem that we identified is the scattered pustule on her face which is commonly called acne. Adult women may present with acne involving lower face and neck is often associated with premenstrual flare and they seem to benefit from hormonal therapies (Thiboutot & Zaenglein, 2016). This condition is also present with Ms. Jones of which she claimed that her skin has gotten better when she started taking the birth control
