The OT will look at how things may have changed for Taylor after a TBI and SCI and then recommends different ways to do tasks or provide modified equipment to support him to be independent as much as possible. This includes recognising levels of care and help needed to complete tasks. PT should conduct both the physical assessment and planning of physical therapy. They can assess and promotes mobility, movement, muscle strength, coordination, balance and stamina. PT can also provide information on walking aids, develop a fitness/mobility plan and provide training to Taylor’s family/caregiver.
I enjoyed reading your discussion post regarding bedside reporting and agree bedside reporting is an excellent example of evidence-based practice. The SBAR tool was originally developed to improve communication between nurses and physicians (Cornell, Yates, &Vardaman, 2014). Moreover, given the impact SBAR had on nurse-physician communication, bedside reporting utilizing the SBAR tool is now evidence-based practice for nurses (Cornell, Yates, &Vardaman, 2014). Bedside reporting gives nurses and patients’ opportunity to set mutual goals.
Physical therapy assistants are educated and skilled in the domain of therapeutic interventions, however they do not receive as much formal education on the examination and evaluation aspects of patient care. With that being said employing the use of PTA’s can run the risk of violating CMS regulations as it relates to documentation. PTA’s may treat Medicare B patient under the supervision of a credential physical therapist meaning he or she must be on site.3,7 PTA’s can write daily treatment notes which are co-signed by a credentialed PT who has established the plan of care.3,7 PTA ’s however can not perform evaluations or establish plan of cares as well as complete the required progress note for every 10th visit.3,7
Delegation allows physicians to streamline their workload, optimize their time, and concentrate on clinical decision-making and complex procedures while relying on healthcare assistants to assist with patient care tasks that do not require their direct involvement. Allied Health Professionals: Allied health professionals, such as physical therapists, occupational therapists, and speech-language pathologists, may delegate certain healthcare tasks to healthcare assistants. Delegation allows these professionals to focus on their specialized assessments and interventions while utilizing the support of healthcare assistants for tasks such as patient mobility assistance or equipment setup. Healthcare Team Leaders or Managers: Healthcare team leaders or managers, such as charge nurses or unit supervisors, may delegate healthcare tasks to healthcare assistants. Delegation within the team ensures that tasks are appropriately distributed based on workload, individual competencies, and patient needs.

In addition, PTAs require lifting patients, moving the equipments or help them people to walk, stand and turn around. However, I want to do more I want to be able to follow the process from beginning to end and do the evaluations. I want to be able to do the testing and the measuring. I have set out on this journey of health care because I want to help people go from post-operative to back to fully functioning with realistic goals in place. I want to be able to walk the journey with the patients and let them know that they are not on it alone. Through Physical Therapy I feel that patients get more than just rehabilitation of an injury or surgery but they get a change in mindset from injured to functioning independently again as they did before.
Evidence and Evaluation in Bedside Reporting Bedside reporting assist nurses with a chance to improve patient safety and increase patient collaboration in the arrangement of care. There is also less care correlated to inaccurate or deficiency of information because the report process includes actual patient apparition. Increased staff approval with bedside reporting supports teamwork and supports accountability. By associating bedside reporting there is an optimistic impact on the patient and their relatives.
Merriam Webster Dictionary defines evil as “sinful” or “wicked” and defines good as “kind” or “right” (“evil”)(“good”).The contrast of evil versus good plays a large role in Bram Stoker’s Dracula. From Christian symbolism to Count Dracula the good against evil theme is very prominent. Bram Stoker was a sickly child and Stoker’s mother, a charity worker and writer, entertained Stoker with fantastic stories (“Dracula”). This is probably what Stoker used to create the theme of good and evil. The heroes proved they are good rather than evil through their selflessness.

I will need to observe the medication administration record, Control drugs record, generic & brand names documents and risk assessment documents. This is important in order to avoid errors while dispensing a medication. Knowing all this beforehand will enable me know the type of medication written on the prescription and where to get them from (fridge, cupboard or the shelves). This knowledge will promote and help to maintain independence in the appropriate way to handle prescription.
Working full time for the past two years, I have had experience being both a physical therapist aide/technician and even an assistant. I have completed observation hours at Gentilly Physical Therapy and Sports Rehabilitation, where I was a technician to a couple physical therapy assistants and physical therapists. At Gentilly rehabilitation center, I understood the nature of the work by assisting patients through weight training, teaching them the proper way to move around and assisted with recording the progress of each treatment. While observing patients under a physical therapist's supervision, I also had the responsibility of supervising lower level technicians during my support duties. Then the following year I observed at PhysioFit; I had a chance to
The thing I did best today, was, communicating with my patient, and doing his physical assessment. My clinical rotation this quarter for Galen College of Nursing ,is at the VA medical center, in Louisville Ky. The VA medical center is a govt hospital for the veterans.
Recently, time was spent observing and waiting at a doctor’s office. The practice is a small, one doctor general practice. Sign-ins are done on individual slips of paper, and left on the counter, then patients sit and wait until called. In the waiting room, the doctor tried to mitigate aggravation for those waiting by providing a television, a variety of recent magazines, and various medical brochures.
