Assessment of Country's Efforts to Address the Health condition Strength The strength in the Family Health program (FHP) is that it provides a team oriented approach to holistic patient care. This health program utilizes a team oriented approach that involves multiple disciplines in the assessment and provision of patient care. Healthcare teams consist of one physician, one nurse, one medical assistant, four to six health professionals, an community agents. Community agents make home visits to asses for individual risk factors in the home and check on patient progress. The accessibility to healthcare is also improved due to each team being designated a community, or municipality, in which they serve a patient population of approximately 3,500 (Manciko , 2010). The success of FHP has lead to rapid expansion of the program with the number of healthcare teams increased from 4,114 to greater than 29,000 between 1997 and 2007 (Manciko, 2010). The FHP has also placed emphasis on the development of health education to increase treatment adherence. Also, the holistic care provided by the FHP has shown to improve the management of chronic and decrease hospitalizations (Manciko, 2010). …show more content…
