The Mt. Pleasant State Hospital, located in Mt. Pleasant, Iowa. The hospital was opened on February 26, 1861 under its original name of Iowa Lunatic Asylum. It was constructed between the years of 1855 and 1865. The asylum when it opened, became the first asylum that was public in Iowa. Mt. Pleasant also housed alcoholics and drug addicts.
C: Mrs. Mack (MHS) reported Jaheim Brown display physical and verbal aggression. A: At 9:58 PM Ms. Smalls (on call case coordinator) was notified by Mrs. Mack that Jaheim started throwing things around the home, refusing to comply with requests and refuse to communicate with Mrs. Mack. Mrs. Mack called law enforcement after Jaheim broke the bedroom closet door and other personal property in the bedroom.
Spoke to Vivian Ortiz (DOB 6/20/75) who stated that her juvenile son (16 years old), Elijah Perez (DOB 7/3/99), was being destructive and threw things around the house when she asked him to turn off the oven after he used it. Ortiz stated Perez had been diagnosed with intermittent explosive disorder and he had not been taking his medication or to his therapy. Ortiz also mentioned that she opened the case with DYFS to evaluate him and the case was still opened. Ortiz stated that Perez threw some baseball cards, laundry bin, trashes and made mess inside of the refrigerator. We weren’t able to speak with Perez as he walked out of the house prior to our arrival.
2:00PM TC Tracie Hope OT from Missouri First about Ms. Virgie Lee Cooper. Tracie stated that she has only worked Ms. Virgie for a short time and she lives alone. There is no caregiver in the home and Virgie does her own housework and cooking until she slip on a rug hurting her leg/no fracture. Tracie commented Ms. Virgie daughter has been concerns about her not eating and paying her bills on time. Tracie explained Ms. Cooper stated her daughter is always in her business she can take care of herself and don 't need anyone but God.
Officer Perez reports being dispatched to a disturbance at 11 Langton. Perez arrived on scene and met with Ann Mcelroy that stated she lost her phone and found that 2 kids had possession of the phone and was able to get the phone returned to her. Mcelroy stated she wanted to make a report of her phones information had been erased and was concerned of any personal information that was possibly had been taking prior to her getting her phone back. Perez had Mcelroy complete a witness statement form.
Lisa do not take Jackson away from Bonnie and Donald’s home; she’s aware that it’s not safe for Jackson to be around the drug use. Lisa has stolen things from Bonnie and Donald’s home, but has forgotten her behaviors. Lisa has threatened to take Jackson away from Bonnie and Donald, which has prevented Bonnie from reporting the allegations previously. Within the past 6-8 months things has gotten worse with
I responded to 500 Trojan Way in reference to a harassing communications call. Upon arrival, I made contact with Amy Evans. Evans stated that her son(Richard Davis) baby mama keeps texting her phone, threatning her. Evans showed me the text messages which stated the offender(Queisha Carlton), was going to stab her son and slash the tires on his vehicle next time she seen him. Carlton also stated that she was going to bust every window out.
On October 5th, as we went to Tripler Army Medical Center, I was placed in the Imaging ward. I got to see all the different types of imaging machines such as Ct scan, MRI, Ultrasound, and Nuclear Medicine. I observed the nurses doing these with some patients. The first patient had to do a CT scan to see what was wrong with their breathing, they had dyspnea. When the nurses was finished looked over it, they sent the images to the doctors so they could say what was the problem.
Mr. Earlington was familiar with Kyree Dobyns and his mother. There were no prior incidents involving this subject. I called Keisha Dobyns (mother) and spoke to her by phone. I confirmed that Kyree was late coming home on October 15, 2015. I asked Keisha Dobyns if she confirmed that her son was in fact at work on October 15, 2015.
In the quiet town of Florida City a robbery took place at Seminole Bank. The robber wore a mask, carried a gun, and got away with $20,000 in cash. Witnesses were unable to identify the robber by his physical appearance because he was wearing a mask. However, the witnesses recognized his voice and identified the robber as Mr. Smallwood. In the case of Smallwood v. State, Mr. Smallwood was accused of armed robbery of Seminole Bank in Florida City, Florida.
Jessica Gonzales, Jessica Gonzales claimed that the town of Castle Rock and its officers “knowingly failed to perform their duties to [respondent] and the three children to protect them from, arrest or restrain Simon Gonzales, and such failure constituted a denial of the due process rights of [respondent] and the three children” (Clement, Keisler, Elwood, Singer, & Scher, 11). In an interview for 60 Minutes, Castle Rock Police Chief Tony Lane explained that domestic disputes are often difficult to sort out. “What safer place can children be than with one of the parents, the mother or the father? And we had no indication from past records that he was ever violent with these children, or even his wife, physically.
On this date worker spoke with Ms. Rosalita Thomas at Meadowview Nursing Home in Bessemer. She stated her father Mr. Roosevelt Thomas was at the nursing home receiving 21-day therapy due to muscle weakness, malnourishment, and dehydration. According to Ms. Rosalita, her father called her on June 30, 2016 and asked her to come get him. When she arrived, she asked to take her father out to lunch. According to Ms. Rosalita Thomas, Donnie was intoxicated and would not allow Mr. Thomas to leave with Ms. Rosalita Thomas.
On 5/28/15 worker visited the home of Ms. Toni Perkins, for the purpose of getting Ms. Gertrude Carter and Floyd Carter removed from the home during the pick-up order on Ms. Victoria Carter. Worker asked Ms. Perkins to call Ms. Gertrude Carter and ask if she and Floyd would go with her to Dollar General. Ms. Perkins became very emotional and stated "I can 't lie to my mother. " At this time, her husband entered the room and stated, "Do something because they don 't need to be around when that happens."
On 07/18/2017 I, Deputy Daniel Pruitt was dispatched to 13535 South 177th West Avenue for a larceny complaint. I spoke to Angela Richison who made the following statement. Angela said on 07/17/2017 she had her house cleaner Leah Wicker come to clean her home. Angela said Leah has been cleaning her home for the past four years.
Before taking this class, I had never seen a CT scan or machine before. I chose to take an on-site visit for this project because of that fact. I visited the Dodge City Medical Center for my on-site visit. While there I was able to see many different things that I had never seen before. I saw many different CT scans specifically for this class, but I was also able to see an ultrasound of the abdomen.
