On 1/17/2016 SO EMT Perez was dispatched to HG-407. SO EMT Perez knocked and announced his presence at the door. SO EMT Perez was greeted at the door by the resident. The Resident, a Mrs. Elsie Cooperman answered the door and was activly bleeding from her face as she was trying to explain what had happened. SO EMT Perez immediatly began to controll the active bleeding and had Mrs. Elsie Cooperman sit down in a nearby chair while he tried to also calm the resident down so he could get some information about what happened.
On 10/29/2015 SO EMT Perez was dispatched to PV-119 regaurding foot pain. SO EMT Perez knocked and was verbally greeted in by the resident a Mrs. Suzanne Truss. Mrs. Suzanne Truss was very stressed and seemingly overwhelmed and she stated that she had foot pain but before SO EMT Perez had a chance to evaluate Mrs. Suzanne Truss got up on her own strength from her bedside and proceeded to walk to her bathroom with assistance of her walker. Mrs.Suzanne Truss wanted SO EMT Perez present in the bathroom because she stated it made her less nervous. After Mrs. Suzanne Truss finished in the bathroom and walked to her bed SO EMT Perez began an assesment which revealed the following; Blood Pressure 110/72, pulse rate of 75bpm, and foot pain of the

On 8/2/2015 SO EMT Perez was dispatched to FC-505 regaurding general sickness. SO EMT Perez knocked and announced his presence at the door and was greeted by the residents spouse and then to the resident in question, a Mrs. Barbra Myers. Mr. Myers stated that Mrs. Myers was feeling unwell for the past couple of days but today she was acting unusal and slurring/mubling her words and all around acting uncoordinated. Mrs. Myers seems to be somewhat disorientated but is answering all questions appropriatly. SO EMT Perez performed and assesment which revealed the following; Blood Pressure 148/72, Pulse 101, Spo2 96%, Mrs. Myers is very hot to the touch but does not appear to be perspiring, SO EMT Perez recieved a temperature reading from Mr. Myers '
On 01-07-16 at about 0955 hrs., R/P Dominguez, noticed that Chavez was missing from the facility. R/P Dominguez advised me that Chavez has been reported missing on several occasions (under case #15001276, 15012422, 15013228). MHP took the member home once his prescription was filed. The member states he forget to make follow up appointment with the doctor for next month. MHP told the member she will schedule follow up appointment and will let him know the date and time. The member report that he still want to find a senior citizen apartment. MHP told the member that he will be responsible for paying for the first month rent and security deposit.
The counselor called an ambulance to rush Sanchez to the hospital because she needed an immediate psychiatric evaluation. The hospital employed a standard for admission to its psych unit that boiled down to a simple question: do you feel suicidal or homicidal? To that, she responded “no”. Just 11 minutes after her psych evaluation ender, she was discharged from the hospital with the name of a clinic she could contact for outpatient services, though she was given no address or contact information. She never made an appointment.

The instructor limitations in being absent too much, errors in work, and inability to stay on task because of talking too much. Human Resources Meeting On Thursday, October 22, 2015 at approximately 8:15am Security manager Mark Warren and Human Resources representative Victoria Micheletti met with Neurophysiology Technician Pepi Dakov regarding his action with hospital equipment. Mr. Dakov stated he accidently removed one NYP Trex Head Box from the Neurophysiology clinic and after several days he returned it to his supervisor Eugene. He also admitted working for Dr. Frid and treating one of her patients but not with NYP equipment.

Mr. Charles Holloway asks me to go with him to 3 Medical Park Garage to discuss the parking issue with PHR’s staff members. Upon my arrival to 4B, Mrs. Tammy Sutton drove up at a fast speed and parked. She got of the vehicle and grab Mr. Holloway wrist then walk over to Mr. Murdock and Huggins. Mrs. Sutton stated that she park in the resident space for over two years and never been an issue, she stated she was the on call nurse and her times at work vary. Then Mr. Holloway explains that she cannot park in patients, visitors, Physicians, and Resident park space any time.
