Nursing assessment is a fundamental part of the nursing role across all fields of nursing. Carrying out nursing assessment provides a baseline of factual, professional and safe care for all patients which can therefore lead to effective wellbeing and diagnosis of patients. Nursing assessment is part of the nursing process. This consists of 5 steps which is assessment, diagnosis, planning, implantation and lastly evaluation. This can also provide adult nurses with a framework of an effective care plan.

Nurses continuously assess patients throughout their time in hospital to see if there is any difference or progression of their patients. An example of an assessment tool that is used in adult nursing is the national early warning score system. This tool is based on a scoring system of which your six physiological Respiratory rate, oxygen saturation, systolic blood pressure , pulse rate And level of consciousness . This means that assessment is part of all the activities nurses perform on patients and also contact eight patients. The purpose of it is to promote holistic nursing care, help make decisions about patients more easier, to organise information collected from the assessment, identify health problems with patients and lastly deliver the best care needed for the patient.
The types of assessment that is carried out by nurses are Initial assessments, emergency assessment, focus assessment and time lapses assessment. Initial assessment is also known as admission assessment and it is taken place when patients first come into contact with the hospital. This is to assess the patient’s health status when they first come in. This also gives the nurse a broad database which is needed to evaluate the patient’s health status
