MercyHealth Willard is one of a seven-hospital system located in Ohio, and works to improve the health of those in the rural area that are under-served. MercyHealth Willard is a Catholic faith-based institution and prides themselves in providing holistic patient-centered care to those they serve. MercyHealth Willard is a critical access twenty-bed community hospital that provides emergency services, inpatient and outpatient care, along with coordination of services. There is only one 20-bed inpatient floor that encompasses four intensive care rooms, with the being telemetry/general/swing-beds. On March 15, 2016 at 4:34 am, I was notified by Elisha Smalls that she had discovered red spots on her arms and mid back and had an itch. The employee was instructed to leave the facility and see her physician as soon as possible and to follow-up regarding the visit. At 11:14am, the employee called back and stated she had chicken pox and was in contact a resident in ALF that had shingles. It was discovered that the employee di d work with the resident on 02/24/2016 and 02/25/2016.It was discovered on February 24, 2016; a resident in Assisted Living did exhibited signs and symptoms of possible shingles. The resident was seen by the Nurse Practitioner, Jody Eblen for a rash with pain, the resident was started treatment with antibiotics on
On 09/29/2015 at 3:18 PM SC received call from Marguerite Pa’s niece who reported that Pa has identified two girls Gabby and Yajaira he wants to be his aide but the agency told them his service were terminated. SC attempted to explain why the service was interrupted and next step; SC end call with Marguerite. SC then called Vital Support and briefly explained to receptionist and requested to speak with a manager. SC was transferred to Vitaliya at Vital Support Nurse manger. Vitaliya stated that the termination date is incorrect and service was provided to Pa beyond termination date and that SC need to change the dates so that they can receive payment.
