On June 18th 2016 Deputy Akins was requested by dispatch to make phone contact with Antonio Johnson regarding Nia Sockwell who had fallen on Tuesday and hit her head on a towel rack. Upon making phone contact with Antonio Johnson, Mr. Johnson explained that his cousin Nia Sockwell, had fallen and hit her head on the towel rack while at her boyfriend’s home Tuesday, June 14th, 2016. Mr. Johnson said the cousin’s boyfriend is “Al” who lives at 1304 Valentine Rd Jacksonville Arkansas. Mr. Johnson could not provide any other information on “Al”.
When inside the store the caregiver and her boyfriend approached the resident appearing upset and curious as to ask why the resident wasn 't still in the truck. The RP disclosed that the resident was fearful due to the tone of voice the caregiver used when asking why the resident left the vehicle. The resident disclosed that she and another resident had traveled to multiple placed with the caregiver, her boyfriend, and daughter (name not provided). The RP stated the boyfriend had not received a background clearance; however he was transporting the residents in his vehicle. The caregiver disclosed the boyfriend had transported her and the resident on at least 3 other occasions and therefore the residents should not be nervous in his presence.
On September 6, 2015 at approximately 8:21pm I, Deputy Tucker George, checked en route to the Shell Gas Station in Hawkins, Texas 75765, Located at 102 East front street in reference to an assault call. Upon arrival I located the complainant Michele Turner inside the Shell station sitting at a small table on the North wall of the building just inside the door. Michele Turner appeared to be intoxicated upon my arrival and displayed multiple signs of intoxication throughout the process of conducting my investigation. Michele Turner was unable to provide detailed information in regards to her assault, but did state the house at which she had been and was assaulted was a known dope house. I later located two small clear plastic bags in the
R/s Vickie stated she found her mother in feces and her father locked her out of the home. ALLEGATIONS: R/s Phyllis Fisher is in a wheelchair and has hypertension, weakness, COPD, O2 Dependent, anemia, and a history of alcohol abuse. R/s Mrs. Fisher lives with her husband, but he can’t take care of her. R/s Mrs. Fisher has been found in her feces.

On 04/14/17, at 4:30pm, I Deputy Warden N. Christian was dispatched to the area of Carnation Drive and Blue Bonnett Drive on an injury-possible dangerous or vicious dog to a minor. I arrived at the location and was met by dog owner Christine Belcher and Blendon Township Police Department (BTPD) Detective Sergeant E. Moynihan. I asked what happen, Ms. Belcher stated that her daughter Daisha Decree were walking their Rottweiler name Bolt when victim Victor Schuetz asked to pet the dog, Ms. Decree give permission. Mr. V Schuetz started petting Bolt on the underside of his neck and move to the top of his head where he was bit.
is a 49-year-old, divorced female, who was employed as the manager of a local firm. She had previously expressed a desire to who additional security on-site, because she often felt unsafe. At around 6:00 AM on the day of her assault, a stranger violently physically attacked her, with both blunt force and with a knife, stabbed her several times, and attempted to sexually assault her, before running off. Andrea suffered several physical injuries, including “a fractured skull, fractured nose, facial lacerations, and multiple stab wounds,” (Butcher, Hooley, & Mineka, 2013, p. 117). Following the incident, and during the assessment, Andrea also presented emotional and mental distressing symptoms.
Case or Issue Overview A woman’s body was discovered floating in the pool located in the apartment building she owned and lived in. Police questioned all of the current tenants. All of them were cleared of suspicion except for Ella Rogers, a 23-year-old publicist. After further investigation, police found some evidence that contradicted her alibi.
Mrs. Jane La Pagilia is being discharged from Prince Albert Hospital after being treated with pneumonia, renal failure secondary to dehydration and dementia. She is now progressing well and will be ready to be transferred into your care. Regarding her medical and social history, she lives with her husband who is 80 years old and is the one taking care of her. Due to old age her husband can no longer provide care which resulted to both having poor health and sanitation. During her confinement, she was initially in a coma and eventually contracted with nosocomial pneumonia which was managed by the physio therapist and is now non infective.
Valerie has babysat the children this last week and stated during her tenor she’s witnessed the children hit, pushed, screamed and yelled at, cursed, and present when drugs are used by their mother. According to the reporter, Kathy hits the children whenever she’s upset an on 08/27/15 the reporter witnessed Kathy hit and push Allison down after the child walked into the restroom while her mother used drugs; the children have not had injuries that require medical treatment. Valerie is unsure what kinds of drugs were used, but she believes Kathy is shooting something up her arm. Kathy has also been witnessed trading her car for drugs. Valerie did not hear any dialogue but witnessed Kathy take drugs from an unknown male and he took her car in
R/s Glenda Baker is totally disabled, she can’t walk long distance and she can’t stand for long period. R/s Ms. Baker has two crushed discs above her tailbone, three bulging disc in the back of her neck, Fibromyalgia, and her spine is shape of an S. R/s she suffers from high blood pressure, high cholesterol, severe depression, anxiety and panic attacks. R/s Ms. Baker takes medication for all her conditions. R/s Ms. Baker is living with her daughter, Lindsay and she is mentally and emotionally abusive to Ms. Baker. R/s Lindsay curses at Ms. Baker and calls her names like “a fucking bitch.”
Courtney stated she got upset with the subject and started yelling at him. Courtney then stated the subject started pushing her and throwing things such as a chair, and a bedroom door. Courtney further stated the subject then assaulted her and pushed her towards a living room window causing the window to break. Courtney continued to state the subject left the incident location and shortly returned to gather his personal belonging’s out of the residence. Courtney stated the subject took her personal blanket out of the residence with his personal items.
