Susan Giles is a 32 year old woman who suffers from an anxiety disorder called Agoraphobia. She is a mother of two kids, a wife, and a good friend to many people. Susan loved her life, but soon after moving to Chicago for her husband’s job, her world turned upside down. She had everything she wanted and needed, but started experiencing physical symptoms such as tingling in her legs, nausea, and a feeling of disorientation. She would sit and stare for many hours. Susan was afraid to leave her house for fear of losing her mind in the middle of the grocery store, or at school when around other parents. No one in her family understood what she was going through. A psychiatrist diagnosed her with panic attacks related to Agoraphobia.
Susan’s relevant functional issues; She was overwhelmed with simple daily functions, such as going to the grocery store, school activities for her children, or attending work functions with her husband. These functions were causing her to have physical symptoms, tingling in her legs, nausea, and disorientation. Her world appeared grey and lacking of colors and light. Susan had no desire to ever leave her house.

She didn’t want to leave her house or be around people. Susan couldn’t do her own grocery shopping because of the fear of having a panic attack in the middle of the store. She worried about letting her husband and kids down when they needed her to attend their functions.
As her therapist, the goal I have set for Susan is to be able to do her own grocery shopping. I have set a goal that she will spend 1 hour 2 days a week working on breathing techniques for her panic attacks, and gradually exposing her to the grocery store which she fears of going in. In 2 months Susan will be able to do her own shopping, and use breathing techniques, or relaxation measures that she has learned if a panic attack starts to
