INTRODUCTION: This case involved Gabriel Walton being a danger to himself and others. Walton was transported to Exodus Urgent Care, where he was placed on a WIC 5150 hold. INVESTIGATION: On 11-10-17 at approximately 1110 hours, LA County Clinician Vasquez #433353 and I was responded to the intersection of Colorado Boulevard and Bonnie Avenue, regarding a mental health evaluation.
Attempt CC =1 CM spoke to Johnda Neazer (DCP&P Supervisor) regarding the family’s current living situation. DCP&P supervisor provided CM with a detailed background on the family and noted that DCP&P will assist the family with housing such as the first month rent and security deposit once caregiver finds housing. DCP&P supervisor explained to CM that the division is unable to place the family into a hotel due to Welfare offering the family to stay at a shelter in Newark and caregiver denied offer. CM was informed that DCP&P only place families in a hotel if they are homeless and does not have assistance such as shelter opportunities. DCP&P supervisor stated DCP&P assisted caregiver with a budget form.
CCIB Intake received the SOC 341 for resident Clothide McCord. Reporting party Tonya Hill Conservator- Deputy Public Guardian 222 W. Brookside Ave., Redlands CA 92373 (909) 798-8500. The reporting party and Public Health Nurse, Sandra Garcia visited Clothide at the facility on 12/13/16. The resident was found barely responsive and dehydrated.

CCIB received a referral #0424-8590-7518-9033739 via fax from Juana Torres regarding Kenneth Salazar (3) (DOB 5/27/2013). RP reported on 3/14/17, Kenneth was physically abused by his teacher 's aide Rosa Espinoza. RP reported she picked him up Kenneth from his Headstart program and took him home. RP reported Kenneth was wearing a sweater and the she asked him to take it off and he did.
CCIB Intake received a cross report call from Marisa Romero with the County of San Diego CPS. Marisa stated she met with Detective Elizabeth Hundley to initiate the investigation of Gabriel Munoz DOB: 4/6/15 head injury. Marisa stated she and the Detective have statements from teachers regarding the incident. The Director was also interviewed and denied the incident happened. The Director continued to deny the incident even when the teacher Monika Huynh admitted the child hit his head when she fell.
CCIB Intake received a call from Co-Complainant Adrienne Williams (323) 802-5315 who disclosed that the son of resident Eleanor Lanzarotta in room 204 punched resident David Lester in the face. Notably David is described as having dementia with aggressive and combative tendencies. Today, April 24, 2017, David entered Eleanor 's room and struck her which resulted in her son striking him back. The complainant was not aware if David sustained an injury from the encounter due to it being the end of the shift. The complainant reported David is not appropriately placed in the facility and poses a significant risk to the other residents in care.
Judge Iglesias scheduled the next court hearing for 1/13/17 at 9:00am. Judge Iglesias wants a biopsychosocial evaluation done by CMO. CM will follow-up with the biopsychosocial evaluation for youth. Substance abuse: Caregiver reported that youth have not smoked marijuana since he has been admitted to YMCA Shelter.
Mrs. Navarro asked the CP if the CP can provide her with information about entering the shelter, at which the CP agreed to do so. Mentoring Program: The CP provided Mrs. Navarro with information about SCR and mentoring programs for Tyler to explore. Mrs. Navarro reported that she enrolled Tyler and Sidney for the PAL afterschool program, but Tyler refuses to attend-so, the child’s application is placed on hold, according to Mrs. Navarro. Tyler was questioned as to why he is not going to the after school program and based on the child response, he could not provide a proper answer.
CCIB Intake received an incident report from reporting party, Cesar Daproza Case Manager. It was reported that on 6/14/16 at approximately 9:30AM Activities Coordinator Karee Montesanti was driving the van south on Lakewood Blvd when another vehicle swerved into her lane and hit the van knocking off the on the rear passenger side. The driver immediately parks the car and checked the clients for injuries. No injuries were noted. The driver than call 911 reported the incident and reported the incident to the Program Director Gustavo Palomares.
The representive stated a packet of paperwork with mailed to patinet in 2-3 weeks to be completed and mailed back within the deadline date. They stated a social worker for IHSS will contact patient to schedule an evaltation appointment with patient. Patient understood the MSW instructions about looking out for the packet and contact MSW if futher assistance is needed. Patinet had other concerns about hearing from Medicare regarding not receving the completed Medicare Coverage Re-determination packet, which was due by 09/01/15. Patient stated she never received the packet through the mail.
On 10/26/16 CM went to unit #222 due a complaint that water was coming from that unit causing some damages to the ceiling in unit #122, situation that has been happing before and was fixed by the maintenance personal. CM ask Ms. Footman was happen she say that anything was fine in her unit, however based on CM observation , the bathroom tub was wet , signs that someone recently took a shower but the floor was dry. Ms. Footman denied having any flood in the unit at that moment. On 11/27/16 CM went to conduct the Safe Sleep inspection with the DHS Compliance Analyst Jeanette Chirico, while in the second floor we went to unit #222 assigned to Ms. Tiana Footman and her three children.
This is for documentation as to reason why my daughter Cadence Lookabill has had so many unexcused absenses. Cadence has struggled with chronic allergies for years and with those allergies she has a lot of mucus which causes vomiting and diarrhea. I have taken her to the doctor for these issues and they just send her home saying there is nothing they can do, so I didn’t take her on those days. If she had strep throat or any other illness I did. Cadence was given notes for the dates in question, she did not get the notes to the appropriate person and after she failed to give them those notes she threw them away.
