Cognitive / Function Assessment Report The goal of the second visit to my client was to do a comprehensive functional assessment, which is physical and cognitive function, including independent performance of basic Activities of Daily Living (ADLs), social activities, or Instrumental Activities of Daily living (IADLs), the assistance needed to accomplish these tasks and sensory ability, cognitive and capacity to ambulate. I started my assessment with the Lawton Instrumental Activities of Daily Living to assess more complex activities necessary for functioning in a community setting, for example, shopping, housekeeping, cooking, managing finances, transportation and taking medications (King 2015). In fact, cognitive functioning is an intellectual activity that leads to knowledge, including all means and mechanisms of acquiring information. According to Barbara King (2015) it involves reasoning, memory, and thinking. It is important to detect specific pathologic illnesses such as dementia and delirium; it also regulates an individual willingness to learn and capability to make decision (p.19). We start up with the Geriatric Depression Scale: Short Form tool to identify my client level of depression. This tool has 15 item and easy to administer and score, since the score goes from 0-5 normal and >5 is suggests depression and > or = 10 is always indicative of depression. My client scored 2 according to his answers, which suggest that my client is normal and do not need further assessment. To confirm my findings, I used the Mini-Cog, which is an assessment instrument for cognitive impairment that combines a 3- item recall test with a clock-drawing test (Doerflinger, 2007). It took me 3 minutes to administer it, and it does no require special equipment, and is relatively not influenced by level of education or language variations. In fact, I told three items to my client and asked to repeat back and remember those three items. Then I asked him to draw a clock face with all of the numbers, and next I asked him to draw in the hands of the clock to indicate a certain time. After the patient has drawn the clock face, I asked him to repeat back the three items that were previously stated, which my client did without problem and gain the score of 3.
