Reflection My current role as a discharge coordinator places me in a unique position. I am employed by Children’s Mercy Hospital through the Pediatric Care Clinic (PCC); however, my office is located in Truman Medical Center (TMC). My colleague and I co-share the position of discharge coordinator, we work Monday through Friday. This schedule means that routinely newborns will be born and subsequently discharged over the weekend. In order to ensure that these newborns had follow-up care, we would block appointments for the residents to assign to them before discharge. I would come in on Monday morning and schedule these appointments in the dedicated blocked slots. The issue I will discuss involved appointments being used that were blocked for newborns that were born and discharged over the weekend. An issue developed where blocked appointments were being unblocked and used by the contact center over the weekend. This meeting gave all parties an opportunity to present their perspective of the issue. “Disciplinary expertise is maximized when members of the interdisciplinary healthcare team can routinely employ strong relational skills and effectively coordinate their work with others” (Weiss, Tilin, & Morgan, p.2014, p. xxiii). Resolution The interdisciplinary meeting and subsequent follow-up meeting achieved a successful resolution. The appointments for the newborns would continue to be blocked; however, an alert would be added to the blocked appointment that only I or my colleague could use or unblock them.
