In this domain the White City VHA scores range from ones to fives. Two of the three elements scored a five, and one element scored a one. The VHA has a clear statement and commitment to PFCC. A model, Health for Life, was developed by the Office of Patient Centered Care (OPCC), which focuses on the whole person. Veterans health care goals, not the disease, are at the center of the care.
By developing shared missions and defining specific tasks, the team establishes clear goals. The sharing of relevant information relies on collaboration with other healthcare team members. After the team’s discussion, decision-making takes place. Better patient outcomes are encouraged through the improvement of the work environment by IPCP. Inter-Professional Education (IPE) in the healthcare context focuses on health professionals and students learning with, from and about one another to improve collaboration and the quality of patient care (Thistlethwaite, 2012). It is of great importance that all the three conditions are met in order to have inter-professional learning and education rather than having students just standing together in the same classroom or other settings. IPE generates a collaborative learning environment in which educators and learners, coming from different professional backgrounds, interact and in which they develop knowledge, skills and attitudes that will be of main importance during team practice (Buring, et al., 2009). The collaborative practice allows finding shared and more creative solutions to the problems faced, it is a constructive process supported by the insights of an entire team (Gray, 1989).
Working in a hospital setting with a team that has members from many disciplines can sometimes lead to issues that are easily solved if only they are acknowledged. Collaborative nursing is on a rise and nurses are providing patient care in a more team-based environment. Collaboration is defined as “a complex process that builds on communication” (pg. 190). Poor communication and lack of respect between physicians and nurses lead to harmful outcomes for patients. Interprofessional collaboration is the key to achieving success across education, leadership, practice, and better patient outcomes. Often times in the hospital where I work, we huddle with a doctor, social worker, case manager, and charge nurse to review the patient and discuss patient care.
When trans-disciplinary teams are used in health care, providers from multiple disciplines cooperate and share ideas from the beginning to create a total health care plan that covers all necessary diagnoses and treatment for a patient. The importance of teamwork in delivering healthcare services to the patients is growing in the recent years (Tanco et al., 2011). It is difficult for NHS to provide quality and excellent services to patient in the absence of great teamwork. According to Canadian Health Services Research (2006) a healthcare sector that promotes the effective teamwork can dramatically improve the quality of patient care, enhance patient safety and minimise the work pressure. Due to the effective teamwork and management of semi-skilled and cost efficient labour, the greater health care provisions are delivering in the hospitals and homes (Ingram & Desombre, 1999). Nurses’ participation in decision making using the team work practice model improves the quality of care patients receive, increases the patient and family’s satisfaction, and adds to overall nursing contentment all of which increases nursing engagement, retention and recruitment (2013). I feel that the interprofessional collaboration model and the shared governance models are very similar in achieving quality care and improving nurses' work environment, satisfaction, and retention. Collaboration of staff requires communication between health care plan team, problem solving skills, implementation and intervention of a plan as a team, understanding own responsibility, along with understanding each member’s role in the team. Given health care professionals, clarification of each member’s role, is important to prevent conflicts. Pointing out each member’s role helps reduce inner team conflicts, alleviate role uncertainty, build trusting relationships and dissolve professional boundaries. Role clarification is complex.
Many states are experimenting with different models of integrated care to develop the best-case collaboration for patients and healthcare workers. In order to reduce the complications of the current health care system the integrated care offers methods of reducing long-term health cost and create better healthcare outcomes. For example, allowing shared patient medical records will increase efficiency and improve physicians understanding of their visiting patient. Having the full medical access for patients allows health care providers to make informed decisions on their patients overall care. This collaboration allows providers to work together in combination therapy more seamlessly and transparent to have better outcomes.
Team based nursing and medical care is currently a popular form of providing healthcare in primary care and inpatient environments. The ability of a patient to make appointments to one of several health care providers who are all familiar with the patient’s health care status greatly enhances the ease of getting an appointment while providing a collaborative and supporting environment for the health care providers. The definition of work team is “generates positive synergy through coordinated effort” (Robbins & Judge, 2009), is truly evident when team based nursing is optimally utilized. The ability to have multiple fellow staff members who are familiar with the same patients or task allows a decrease in stress and emotional exhaustion when utilizing collaborative efforts to work in harmony (Welp, Meier, & Manser, 2016). Since technology revolution has taken place in many areas of our life, quality of services in healthcare has become one of the most important objectives that concerning both private and public organizations. Because of the relation between work performance and patient care, according to [Aiken et al. ,2002] there are many hospitals have poor services quality which has impact on patient treatment as a result of frustration of teamwork. Also, they concluded that a key of improving services in health environment is cope and solve nurses’ job dissatisfaction.
There is agreement between team members to put their best effort in facing challenging, problems, and also meeting the team objectives. There may be collaborative cross-team and cross-boundary working. Shared leadership in practical enable members are fully involved in appropriate decision making. This responsibility should be delegated to members appropriately. Also, constructive debates are welcomed to provide and improve high quality patient care.
