INTRODUCTION:
This case involved Jason Wayne Lane being a danger to himself. I transported Lane to the Olive View UCLA Medical Center where he was placed on a WIC 5150 hold.
INVESTIGATION:
On 08-02-17 at approximately 1150 hours, LA County Clinician Vasquez #433353 and I responded to the above location regarding a psychological evaluation. Witness Alma Sainz advised Pasadena Dispatch that Jason Wayne Lane was inside of the location and felt suicidal.
Upon our arrival, Clinician Vasquez and I met with Lane, who told us the following information: Lane was feeling depressed and had suicidal thoughts. Lane did not specify how he would kill himself. Lane has been diagnosed with being Bipolar/Schizophrenic and has not taken his medication. Lane has attempted to commit suicide in 1992 by trying to overdose on Heroine. Lane also attempted to commit suicide in 1994 by cutting both wrists. Lane has been addicted to crystal methamphetamine since the age of eleven. Lane stated he hasn’t slept in the past seven days.

Sainz told me the following information: Sainz was approached by Lane, who told her that he felt suicidal and was planning on hurting himself. Lane reportedly did not specify how he was going to kill himself.
Based on Lane’s and Sainz’ statements made to us, Clinician Vasquez formed the opinion that he was a danger to himself. We placed Lane on a WIC 5150-Mental Health Hold. I transported Lane to the Olive View UCLA Medical Center for evaluation. I informed Doctor Benitiz of the circumstances.
While inside of the medical center, Lane told RN Sheryl (747-210-4341) that he wanted to kill his wife by shooting her. Lane then planned on running outside of his wife’s residence with a gun in order to have the police shoot and kill
