A care plan is what is widely used in the medical field to create a program that is specified for that patient to follow and re-evaluate for its effectiveness. This provides a plan for the next course of action and allows other healthcare staff personnel overseeing the patient to follow the objective to get to the goal that is set. As a nurse, I have encountered many care plans for my residents/patients. It’s like following a cookbook recipe. This care plan continues to be reassessed, added, removed, or slightly changed. This writing was targeted at those in the medical field such as nurses, doctors, and other professionals that are involved in the patient’s care. In this text by Matt Vera, he wrote what a nursing care plan is, its purpose, how to create one, and its outcomes. For example, a patient comes into the clinic that requires medical help. He/She has been overseen by doctors and a care plan was created.
