Ms. Martin is a 14 year old female who presented to the ED after an altercation between her mother and she. She denies suicidal ideation, homicidal ideation, and symptoms of psychosis to nursing staff. At the time of the assessment Ms. Martin is calm and cooperative. She reports tonight her mother would not let her eat a pop-tart for dinner and she became anger and proceeded to hit her mother. Ms. Martin states she only gets angry at her mother and lashes out at her. Per documentation Ms. Martin also lashes out at her grandmother with verbal and physical aggression. Per documentation LEO was called out the home. This clinician contacted the officer who respondent to the call. Officer Stacker who informed this clinician, Ms. Martin reported having an altercation with her mother because she told her she could not eat the pop-tarts for dinner. He reports she stated to him, "I don 't want to live." Ms. Martin was confronted about this statement and she states, "It 's not like I want to kill myself, I just don 't want to have to deal with my mother anymore." Ms. Martin appears tearful during assessment. She expressed feeling of depression as feelings of hopelessness, tearfulness, Insomnia, sadness, and anger. Ms. Martin Therapist Brittany Murray was contacted informed this clinician Ms. Martin is receiving Intensive-In-Home services, which she has been attending sessions for the past few months since finishing services at Youth Unlimited. Osborne and Howard McQuirter, LCSW. They both share the disposition that the patient should be discharge pending her agreeing to follow up with outpatient services. Patient has agreed to follow up with outpatient services as recommended. The patient 's family mother has been contacted and made aware of plans concerning the patient. Patient mother contracted for safety due to Ms. Martin being a minor, was given outpatient referral information and completed a crisis plan.
