Met with the patient as she was placed on a Director HOLD to address the incident on Sunday. Please note, upon meeting with the patient, she appeared to be upset and emotional. According to the patient, patient #2737, pointed her out in the lobby in front of other patients as her attacker, which upsetted the patient. This writer calmed the patient as she clearly upset about the matter; however, noticed that the patient is still walking around with the box cutter on the key chain as she was advised that this is unacceptable. Patient responds was, " I know...I know...alright..." If the patient continues to bring in that weapon on the clinic 's property, she will not be allowed inside of the clinic. This writer explained the reason for the intent to discharge due to the physical altercation and advised the patient, she can request a hearing to address the discharge. The patient refuses to fight the intent to discharge in writing because her responds was, " I wanted to leave here anyway." This writer was offended by the patient comment and address it immediately as the patient then apologized. Patient, again, was emotional and showed a lot of frustration about the whole altercation. Patient signed the intent to discharge without any hesitation.

This writer escorted the patient to the lobby area and advised the patient that this writer will talk to the clinic 's TEAM to remove the director hold so that she can dose. Upon returning to the lobby area, the patient was seen at the dosing window, dosed, and walk outside of the clinic. This writer approached the patient and informed the patient, the Program Director wants to meet with her to address the appeal process, even though the patient decline the hearing, but because the patient has come a long way to her recovery process. The patient smiled at this writer and says,
