In the past nursing was grounded from medicine, social, physical, and other disciplines. At present time, nursing has its own body of knowledge that is exceptional to nursing itself. Although it’s not always the situation the amount of nursing care evaluation has expanded in the past 30 years. It is a highest level of recognition awarded to a health care organization. One of the requirements is for the organization to provide quality-nursing care through evidenced-based practice (EBP). Research has demonstrated that magnet hospital have improved patient outcome since nursing practice is based on the best and current evidence. Nurses working at Magnet hospitals are encouraged to evaluate and question their own practice and participate in EBP committee. In addition, magnet status attracts quality nurse. Furthermore, highly educated nurses correlates to safe and better patient outcome (Wise, 2009). Trend of hospitals acquiring Magnet status is increasing. I worked at a very busy and fast phased emergency department in Downey. Many patients get discharged from the emergency department without understanding any information needed to care for themselves at home or their family members. Patients have demonstrated inability to understand their medication, homecare and follow-up information. Knowledge deficit in the area of home care instructions were about 80% while the knowledge regarding return demonstration were about 79%. Lesser information deficit was found regarding follow-up, medication and diagnosis. Failures to understand discharge instructions completely play a significant role in patient’s recovery or readmission. Enhanced communication between the nurse and the patient are crucial in providing education teaching upon discharge (Engel, Buckley, Forth, McCarthy, Ellison, Schmidt, Adams, 2012).

Leininger’s theory talks about caring as the essence of nursing and nurses who utilize her theory of cultural care in their practice understand patient in the context of their culture (Chitty, 2005) I had a patient who was brought in by his son. Their religious background is Muslim. His son made sure that only female staff attended to his mother. Even the length of the hospital gown is inappropriate with their belief.
