Attempt CC =1 CM spoke to Johnda Neazer (DCP&P Supervisor) regarding the family’s current living situation. DCP&P supervisor provided CM with a detailed background on the family and noted that DCP&P will assist the family with housing such as the first month rent and security deposit once caregiver finds housing. DCP&P supervisor explained to CM that the division is unable to place the family into a hotel due to Welfare offering the family to stay at a shelter in Newark and caregiver denied offer. CM was informed that DCP&P only place families in a hotel if they are homeless and does not have assistance such as shelter opportunities. DCP&P supervisor stated DCP&P assisted caregiver with a budget form.
CCIB Intake received the SOC 341 for resident Clothide McCord. Reporting party Tonya Hill Conservator- Deputy Public Guardian 222 W. Brookside Ave., Redlands CA 92373 (909) 798-8500. The reporting party and Public Health Nurse, Sandra Garcia visited Clothide at the facility on 12/13/16. The resident was found barely responsive and dehydrated.

Ms. Connell believes Gentiva Hospice reported the allegation to DHR because the bath worker stepped in dog feces. During visit worker noticed two inside dogs but no dog feces. Ms. Connell married three times and each one was in the military. However, due to her income Ms. Connell is not eligible for assistance. Ms. Connell receives 1500.00 SS, Medicare, Healthsprings Insurance and Gentiva Services bath aid 3x and nursing 2x weekly.

The SOC 341 states that Maria disclosed to facility staff and her Day Program staff that residential facility staff member Gloria G hit her in her back. Once notified by of incident the facility staff notified the administrator Kendal Woods (323) 208-0133. According to the RP the incident is under investigation. The suspected abuser would be suspended pending

CCIB Intake received referral #0651-1009-4066-0071107 dated 10/30/16. The reporting party (RP) was Oneil Malcolm, Facility Manager. The RP stated on 10/30/16 while supervising minors Devante Pinson and Jacob Sigala, Devanet was observed taking Jacob 's phone charger and walk towards his room. Jacob was observed stepping in front of Devanet asking repeatedly for him to return his charger. As Devante walked away he moved Jacob out of his way using his left hand.
On the date of 03/08/2017 at about () hours I was dispatched to the address of 300 N 6th St (the Greybull Motel) in Greybull to a reported domestic violance situation. I was informed by dispatch that the reporting party was in the office of the motel at this time. Upon my arriavl at the above location I spoke with the reporting party, Donald MOFETT() who informed me that his girlfriend had struck him multiple times in the face, neck and shoulder. I observed that MOFETT 'S left eye apeared to be swollen, I also observed marks on the left side of his face, and on his right sholder. When I asked MOFETT what had happened prior to the incident he stated that himself and his girlfriend Montica ROMERO() had gotten a motel room to have some time away from her family that they are staying with, and that both parties had consumed alcohol.

I called Geico to request assistance in paying for a rental car while my car was being repaired. The adjuster and I were unable to come to any sort of agreement on the time and place for him to come see the car. This is a Workers Compensation matter referred over by Patrick Quinn. Charlene works as a Nurse in the NICU in the ICU and has been employed by the Hospital for approximately 23 years. On November 19, 2015 beginning at about 11:30pm Charlene was working her regular shift when an outside contractor was changing ceiling tiles in the hallways just over her work area. These tiles had been randomly falling due to leaks in the pipes above the tiles.

P alleges that MOS had stopped all traffic at 42 Street and Lexington Avenue causing his bus to stop. P alleges that while inside the bus he attempted to ask MOS why traffic was stopped, MOS Kane approached the bus and began to yell at P. P states that he waited 15 minutes then exited the bus and approached MOS Kane’s marked vehicle. P states that he began to write down MOS Kane’s vehicle number, which he needed to fill out a report for his MTA supervisors to explain the delay.
Mr. Charles Holloway asks me to go with him to 3 Medical Park Garage to discuss the parking issue with PHR’s staff members. Upon my arrival to 4B, Mrs. Tammy Sutton drove up at a fast speed and parked. She got of the vehicle and grab Mr. Holloway wrist then walk over to Mr. Murdock and Huggins. Mrs. Sutton stated that she park in the resident space for over two years and never been an issue, she stated she was the on call nurse and her times at work vary. Then Mr. Holloway explains that she cannot park in patients, visitors, Physicians, and Resident park space any time.
