Shoreview VA Hospital is one of the VA facilities that solely serves the Miami-Dade County. This county is found in the lower eastern part of Florida, but the facility is found in Homestead, Florida. Nearby, is the Homestead Air Force Base, one of 21 Military bases in Florida (See Appendix A for a map). Demographics. Miami-Dade County, has an estimated population of 2.66 million people according to the U.S Census which makes it the most populous county in Florida.
Background statement: Heritage Valley Medical Center has had a wonderful reputation for providing excellent health care services to their community. Initially, their community was 80% Caucasian, 40% African American, and 5% Hispanic. However, in the last 5 years, the population has changed to more minorities and the whites have moved out to the suburbs. This caused the Center’s occupancy rate to go down 40% because many of their traditional, more affluent, private-pay patients had left the neighborhood. To bring in revenue, they campaigned to bring in more Medicaid patients.
Surgical technology is a profession meant for people who don’t visualize their selves working in front of a computer, spending their whole day with paper works or waiting for the time to pass. This line of work suits well for those individuals who are hungry for action. It is a career made for the brave, the fast and the people who consider themselves as OCDs. Surgical technologists are the “worker bees” of the operating room.

Monday October 26th: Today at Moses Cone hospital, I was in the role of student nurse. I had two patients; one a returning patient, and the other a new patient. The nurse I was working with is someone I have never worked with before or even seen on the floor, but she seemed to be familiar with the unit. It was about 3-5-45 minutes into the shift before we received hand off report. After about 8 minutes of waiting to get report on the rest of her patients, my nurse went ahead and started to see patients.
For the last four months at Berkshire Care Centre as a social work intern, I found that I have become familiar with the procedures and policies of the home, which has helped enhance my understanding of what it is like working in a long term care home. Before, I did not know much about long term care homes or what a social worker’s role is. From working at the home, it forced me to get out of my comfort and do things that I thought I would never be able to do like facilitating care conferences on my own with various health staff present. Since graduating, however, I have been fortunate enough to establish a multi-year shadowing opportunity with an ER physician. This experience has easily been one of my most meaningful exposures to the medical field. Unlike other specialists I've observed, whose practices focus on a narrow branch of medicine, shadowing in the ER has allowed me to familiarize myself with pathologies affecting all body systems. Furthermore, having frequent shifts in the ER, I've been able to transition from a passive observer to an active contributor; I now recognize how certain ailments present, and can intelligently discuss possible diagnoses with my mentoring physician. This experience has enabled me to learn about the human body instead of just the specialty itself.
Reflecting on the past seven weeks I have acquired countless knowledge, which I will use to further strengthen my profession as an Advanced Practice Nurse. The course allowed me to think beyond my current practice knowledge and acquired innovative ways to evaluate the situation at hand. The learning objective in program outcome four helps set standards that I will use to guide my clinical practice to meet various healthcare needs. Using the case studies has helped to further enhance my knowledge on disease physiological state, using differential diagnosis, disease manifestations, and clinical presentation. It has also taught me the skills on how to differentiate between similar diagnoses to properly identify the problem and treat the patients.
Obviously, practicing in one of the most modern healthcare system in Australia, with the clinical placement program, under the guide of the expert supervisors, I will be provided the best opportunity to view the most up-to-date practices in health and medicine in the context of the Australian health system. Inasmuch as 4 weeks is not a long period of time, although, I am always eager to learn, it is necessary to have a list of goals, work 100% of energy and study seriously to optimize my outcomes. It is necessary to readdress taught information as reinforcement will provide an increase in confidence. This clinical experience has really helped me to sharpen my communication skills and realize just how important it is to understand mental health. We are told multiple times in class that mental health issues can be seen on any floor and that is the truth. I’ve seen patients in my older adult clinical on the pulmonary floor suffer from issues that range from anxiety to bipolar disorder and depression. Being able to understand how to approach people that suffer from these types of illnesses, allows us, the nurses, to give the patient the best care that we can. It helps to build a trusting relationship and get to know them on a personal level.
Investing my time in the care of my patient gives the opportunity to not only assist them in a difficult situation, but also to learn more about their diagnosis and the treatment, while comparing it to what we have learned in class. For example, I had a patient that suffered from Sickle Cell Disease and came to the ER during a crisis. Correlating this case to the books and the content learned in class, these patients receive at least 1000 mL of fluids, pain medication, and oxygen. Additionally, I had a patient with meningitis. This individual presented with common symptoms such as nuchal rigidity, muscle pain, fever, and chills.
We also had a very informative and up to date course giving us the latest ways of management and medications. I was astonished by how advanced is this field, interventional cardiology was the part that I really liked and witnessed the importance of how we have to act rapidly to improve the outcomes. I also felt the extreme importance of that since cardiovascular diseases come on the top of the list of the most common causes of death in our community and working hard in this field in treating and preventing these disease can have a huge impact on many
