Late Entry
On 10/22/2015, CM met with the client and completed Bi-Weekly ILP Review. Client was dressed in proper attire for the weather. She was well groomed. In the meeting, client appears to be decompensating. She was sad and depressed. Client report she was upset about the P4P meeting. She continues to report as she quote “she hates everybody and nobody is doing anything for her. She is being doing a lot for herself and she is tired. She also mentioned she will be isolates herself from her family. She continues to report she is not abusing any drug and there is nothing else for her to do. She states that people are trying to find something that doesn't exist and there is nothing wrong with her”.
Client also met for P4P meeting. Attendee: V. Tapping/Program Analyst, M. Rivera-Hernandez/DSS, S. Purnell/Program Manager, M. Goodwin/SSS, client and CM. the following topic was discussed: Client housing plan, client uncontrolled seizures disorder, client non-compliant in accepting medical services, client substance abuse denial and Unrealistic Housing objectives.

Client agreed and signed the P4P form.
CM inquires if the client had any recent uncontrolled seizures. Client replies "yes ". The last seizures disorder was on 10/18/2015, and she was taken to Woodhull Emergency Room via EMS and handcuff because she was refusing medical services. Client also reported she is compliant with her medication. Client report the following medication scripts refill 10/27/2013, dentist appointment 10/28/2015.
CM issued client with an appointment slip to meet with SA/MICA specialist on 10/29/2015 for initial assessment. Client agreed and signed the appointment slip. Client continues to deny any substance abuse
