Sample Clinical Appeal Letter

I am writing this letter to request a grade revision for my final clinical evaluation signed off by Kristen Distasio, RT(T) at Lahey Clinic, Burlington and the re-evaluation of the response given by prof. Ebert.
I would like to start my appeal based on the concerns presented on my initial appeal; also I will be presenting additional information based on the response for the grade appeal letter given by prof. Ebert.
Prof. Ebert response to the appeal denoted many inconstancies that, I would like to address and properly provide documentation supporting my case.
• “You were provided the same opportunities as your classmates who successfully completed the course…the primary method used to teach students basic therapy knowledge is clinical rotation”. Prof. Ebert. That statement shows that

The attendance system was changed this semester to an electronic method, which caused some adjustments and issues to the overall class. I never received any complains for the therapists commenting on my attendance. However, during few weeks in February I had issues with my phone accessing the Trajecsys system for the clock in/clock out. After a meeting with prof. Mendelsohn, this issue was investigated and corrected (see ref. 6). I also got a clinical infraction with a loss of 15% of the attendance due to a violation to copy the clinical coordinator. The email was sent to my clinical supervisor stating my condition. Attached is my full attendance that will indicate my professionalism in terms of attendance. (see ref.7 ) As regard of retaining clinical information, I was asked to perform a re-comp to prove my ability to retain learned information in which I passed. In addition, I did perform other competencies, were it was signed by the therapist stating my ability and aptitude to treat more complex cases. (see
