On 6/29/15, Ms. Wendy Sanders and Mr. Charles Ray Hamilton visited the DHR office. Ms. Sanders is interested in becoming Mr. Hamilton 's rep. payee and caregiver. According to Ms. Sanders, she needs somewhere to live because she recently moved to the Carbon Hill area after getting out of an abusive relationship. She has been cleaning Mr. Hamilton 's home getting it ready for her son who is 13 years old. Worker explained to Ms. Sanders how his grandchildren have caused problems in the past for people who have tried to help Mr. Hamilton.
As you are aware Barry Staley residing at McLees, ICF CCNS-Services for people with Developmental Disability 112-16, 200 Street, St. Albans, NY 11412, has been transferred to Silver Crest Nursing Home, a Long Term Rehabilitation Nursing Facility. As of 10/24/14 Barry is now residing at the Silver Crest Nursing Long Term Facility located at 144-45 87th Avenue Jamaica, New York 11435. Prior to this move Barry was admitted from (name Hospital) from (date) to (date) receiving treatment for (condition). During hospital stay a discharge meeting was held. Subsequently he was moved to a nursing home.
On 09/29/2015 at 3:18 PM SC received call from Marguerite Pa’s niece who reported that Pa has identified two girls Gabby and Yajaira he wants to be his aide but the agency told them his service were terminated. SC attempted to explain why the service was interrupted and next step; SC end call with Marguerite. SC then called Vital Support and briefly explained to receptionist and requested to speak with a manager. SC was transferred to Vitaliya at Vital Support Nurse manger. Vitaliya stated that the termination date is incorrect and service was provided to Pa beyond termination date and that SC need to change the dates so that they can receive payment.

As well the person or persons must prove that they cannot afford to reside in a traditional long term care facility, such as a nursing home or assisted living, without incurring out of pocket cost. To be eligible, the individuals must be African American, U.S. citizen, reside in the state of Mississippi, and have a median $650 SSI income. As well they are not required to liquidate there asset’s and must reapply every year to reevaluate their eligibility.

Josepha has been a nurse for four months, and after completing two months of orientation she takes a full assignment as a registered nurse. Josepha feels that the assignments she receives are not always fair, as she tends to get the most challenging clients.
There has been an extensive change in the U.S. demographics within last 50 years. According to Center for Health Workforce Studies (2006), in 2020, there will be about 54 million Americans above the age of 65 years as compared to 35 million in 2000. This number is expected to rise further at the rate of 12.5% to 20%, which means by 2050 every 1 in 5 American will be above 65 years (p. 2). This exhibits how baby boomer generation will have a significant impact on the current long-term care system. Long-term care is defined as the continuum of services framed to support the needs of people with chronic health problems and physical limitations.
WOOSTER — The Wayne County commissioners and Job & Family Services workers wore purple Wednesday to raise awareness about elder abuse. Director Rich Owens and Melissa Plotkin, who investigates complaints of elder abuse, talked about how the number of cases have been on the rise, how abuse/neglect/exploitation cases are investigated and the confidentiality of each investigation. Over the past two years, Plotkin has been investigating between 280-290 cases annually. Which is up over previous years.
The different types of support that may need are as follows: 1. Counselling or coaching: This involves providing personal coaching or counselling skills to the staff. The people who give coaching are trained professionals who have much life experience and they help people to make changes in their work life. If this support is provided to the staff, then there would be less problems and people would also become less resistant to further changes. This means that they would be ready and would accept any changes made in the future.
These facilities can be improved to reach higher expectations and tremendous rewards, but in order to do so, these events taking place in nursing homes and assisted living centers need to stop immediately. One way these facilities can improve is to require further education for the employees to complete so there will be an increased amount of suitable crowds hired to work. These facilities are already unbearably expensive so it would be an easy fix to use part of that money to raise the employee’s pay. Once there is a better group of employees with higher education and better pay, then there should be improvement in the care the residents receive. Superb quality nursing homes start with a superb quality administrator.
According to a Child Protective Investigation, there are approximately half a million children in the U.S. foster care system, otherwise known as congregate care (group homes and institutions). Children are placed in congregate care when they are found to be in an unsafe environment. Usually children of abuse or maltreatment are placed first (Font, 2015). Out-of-home-care causes increased problems of attachment, behavioral, and psychological disorders in the developing child. Child safety is the primary goal of out-of-home-care; however, maltreatment investigations are still reported in those institutions.
Description: In week 7 we had visited Wesburn Manor, the Long-term Care (Clinical Placement) setting. It was our first time there, therefore as a group, we oriented the place. At this time, we went to each nursing station on each floor and introduced ourselves from the organization we were from and how we will be providing patient care to the clients in this setting. We were educated by our clinical instructor on the different access codes in the building, the policies and guidelines, our assigned floor and the appointed personal support worker. Our role of professionalism as a nursing student was represented as we came prepared and greeted each healthcare and staff member.

Caregivers in the unpaid category are generally family members who assist or support their loved ones to perform daily living activities. They are also called informal caregivers. These caregivers may or may not be a certified nurse. But they are matured, qualified, and able to follow instructions of doctors and nurses for patient care.
