CCIB received a SOC 341 from APS for the following residents: Daniel Hourihan (DOB: 10/03/54), Gerardo Guerrero (DOB: 01/02/95) and Gerald Gaither (DOB: 06/14/59). Per the reporting party, Lee Nln, the provider/Owner Cheryl Oliver has been advertising her independent living facility as a board and care. RP stated that her clients have been paying the board and care rate and the client require medication management. RP stated that in April 2018, client Daniel Hourihan moved to another facility. The rent for Daniel was sent to Ms. Oliver who continued to cash the rent checks for Daniel.
Received a report on 10/7/15 stating Ms. McGinnis was not given medication properly and scoring a 0 on MMSE. Son, Michael McGinnis reported he gave her dose of Methadone because she was hurting. Ms. McGinnis admitted to WBMC on 10/6 with chest pain and hypertension. While on medical unit, she was confused, pulling the cardiac monitor off, yelling, screaming, cursing the nurses, disrobing, and refused meds. On 10/7 she transferred to Senior Care she remained until 10/28 3:30.
On 11/4/15 Ms. Brittany Terry and her daughter Lundyn met with CM for an ILP. CM introduce herself as the client temporary CM assigned to her case. Housing: Client 's exit strategy is to move by 2/18/16. Client must maintain employment and submit pay stubs weekly .Client is expected to comply with savings and have saved enough for one month 's rent and security by 12/18/16, Client is to seek for housing and obtain housing by 1/18/16.Client was reminded she must meet with her Housing Specialist Mr. Brown on a bi-weekly basis, comply with all housing requirements and explore all available housing options. Social Service: Client must meet bi-weekly with Case Manager for ILP Review appointments.
Mrs. Jones is a 67 year old female who presented to the ED via LEO under IVC through DayMark Recovery Services. Per documentation Mrs. Jones has been non compliant with medications and has been experiencing symptoms of psychosis. At the time of the assessment Mrs. Jones appears calm and cooperative. She does present with tangential speech and vaguely answers questions. She recently was discharged from Novant health 1/3/17 and followed up with DayMark 1/11/17 with new changes in medications.
SC, Jennifer Stoker contacted provider, Latonia Jennings via telephone. SC introduced herself and told provider that she was Barry’s SC. SC asked how Barry is doing towards his outcome of wanting his health monitor. Latonia noted the nurse is monitoring his health. There has been no issue and he staff monitor his health daily to insure he remains health.

Bits and Pieces Case Overview This case focuses on an employee, Mary Beth Jacobson, the department head for the radiology department, who had been receiving perks from three specific vendors in which the Affiliated Nursing Homes and Rehabilitation Center makes their purchases from. From the case, it is apparent that other employees were oblivious of the gifts that she was receiving. It was not until John Henry Williams discussed his new position with her that it became known. The question that this case conveys involves when does appropriate become inappropriate?
Shoreview VA Hospital is one of the VA facilities that solely serves the Miami-Dade County. This county is found in the lower eastern part of Florida, but the facility is found in Homestead, Florida. Nearby, is the Homestead Air Force Base, one of 21 Military bases in Florida (See Appendix A for a map). Demographics. Miami-Dade County, has an estimated population of 2.66 million people according to the U.S Census which makes it the most populous county in Florida.

The proposed program will address the needs of minorities 65 and older at the time the program is implemented. Those taking part in the program would not loose any of the health benefits they are receiving from Medicaid under the current SSI health benefits insurance program. As well the person or persons must prove that they cannot afford to reside in a traditional long term care facility, such as a nursing home or assisted living, without incurring out of pocket cost. To be eligible, the individuals must be African American, U.S. citizen, reside in the state of Mississippi, and have a median $650 SSI income. As well they are not required to liquidate there asset’s and must reapply every year to reevaluate their eligibility.

The cost of nonadherence is estimated to cost the US healthcare system $100 to $300 billion a year (Phrma, 2015). This figures includes cost of hospitalization, premature death and nursing home admission. Adding an additional $2,000 to the annual per patient cost, 40% of the nursing homes admission has been associated with non-adherence (Phrma, 2015). Socioeconomic factors One’s socioeconomic levels can contribute to negatives outcomes related nonadherence.
Hayes learned that the vast majority of the patients only used Boardwalk Clinic during the times when their current caregivers were unavailable and they planned on returning to their current medical providers during traditional business hours. Thus, patients that need services during nontraditional hours most likely chose Boardwalk Clinic as their second choice, only because their first choice was not available. Although Dr. Hayes and her colleagues had a better understanding of the retention issue, they were still at a loss as to what steps the clinic could take to attract those patients in the Chelsea marketplace who already had relationships with other healthcare providers to make Boardwalk their first choice. There are a couple steps Boardwalk Clinic could take to ensure an increased rate of patient retention.
Description: In week 7 we had visited Wesburn Manor, the Long-term Care (Clinical Placement) setting. It was our first time there, therefore as a group, we oriented the place. At this time, we went to each nursing station on each floor and introduced ourselves from the organization we were from and how we will be providing patient care to the clients in this setting. We were educated by our clinical instructor on the different access codes in the building, the policies and guidelines, our assigned floor and the appointed personal support worker. Our role of professionalism as a nursing student was represented as we came prepared and greeted each healthcare and staff member.
