On 6/29/15, Ms. Wendy Sanders and Mr. Charles Ray Hamilton visited the DHR office. Ms. Sanders is interested in becoming Mr. Hamilton 's rep. payee and caregiver. According to Ms. Sanders, she needs somewhere to live because she recently moved to the Carbon Hill area after getting out of an abusive relationship. She has been cleaning Mr. Hamilton 's home getting it ready for her son who is 13 years old. Worker explained to Ms. Sanders how his grandchildren have caused problems in the past for people who have tried to help Mr. Hamilton.
Spoke to Vivian Ortiz (DOB 6/20/75) who stated that her juvenile son (16 years old), Elijah Perez (DOB 7/3/99), was being destructive and threw things around the house when she asked him to turn off the oven after he used it. Ortiz stated Perez had been diagnosed with intermittent explosive disorder and he had not been taking his medication or to his therapy. Ortiz also mentioned that she opened the case with DYFS to evaluate him and the case was still opened. Ortiz stated that Perez threw some baseball cards, laundry bin, trashes and made mess inside of the refrigerator. We weren’t able to speak with Perez as he walked out of the house prior to our arrival.
Ms. Connell believes Gentiva Hospice reported the allegation to DHR because the bath worker stepped in dog feces. During visit worker noticed two inside dogs but no dog feces. Ms. Connell married three times and each one was in the military. However, due to her income Ms. Connell is not eligible for assistance. Ms. Connell receives 1500.00 SS, Medicare, Healthsprings Insurance and Gentiva Services bath aid 3x and nursing 2x weekly.
10/29/2015 at approximately 1918 hours, this officer was dispatched to 207 Florida Avenue for a mother and daughter domestic. Both Denise and Shirley HAZLETT could be heard screaming while I was outside the home on the sidewalk. This altercation was occurring during Shenandoah Height's trick or treat event with numerous neighbors, parents and children walking around the home. I briefly spoke with several neighbors who were standing outside. They expressed thier concerns about Denise and Shirley HAZLETT.
Mrs. Navarro asked the CP if the CP can provide her with information about entering the shelter, at which the CP agreed to do so. Mentoring Program: The CP provided Mrs. Navarro with information about SCR and mentoring programs for Tyler to explore. Mrs. Navarro reported that she enrolled Tyler and Sidney for the PAL afterschool program, but Tyler refuses to attend-so, the child’s application is placed on hold, according to Mrs. Navarro. Tyler was questioned as to why he is not going to the after school program and based on the child response, he could not provide a proper answer.

The placement appears to be safe and no visible hazards were observed. The minor is provided her own bed and shares the bedroom with other sisters. On the above date and time I was conducting a business check of Spice Asian Fusion. While conducting the business check, I observed a White male laying next to the front door of the property. The subject appeared to be sleeping on a cardboard box. I macde contact with the subject, later identified as Newton Clark Kline III. Clark advisedd me that he was just sleeping and not in need of medical attention.
worked for the Children’s Center of Cicero Berwyn as a part-time Home visitor for six months from September 2016 thru March 3rd, 2017. She was a good employee who had passion to work with children and families. She created lesson plans for five children and families, prepared developmentally appropriate activities, observed children, provided resources and referrals to families in the communities. Her doctor had instructed accommodations and those were in place for her. Those accommodations included not to sit or stand for prolonged periods of time and not to lift more than 10 pounds.
CCIB received a SCAR/CPS referral #0306-4829-5973-3020047. The reporting party (RP) Ashley Shinn reports her son, Aedan Shinn (DOB: 01/10/12), attends the Reyes family Day Care Monday thru Friday. RP reports a couple of months ago, her son attempted to tell her how the licensee disciplines the children when they are not asleep during their nap time. RP states Aedan at first was going to tell her, but then said he would get in trouble for being bad. As recently as yesterday, Aedan finally disclosed to the RP that licensee when checking in on the children taking a nap and notices the children not sleep with their eyes closed she will hit them on top of the head with a closed fist.
On 08/28/15 Steven came to school covered in live bed bugs and bed bug bites; this is the second time this has occurred. Steven told the reporter his grandmother treated the bed bugs on Monday but there is still an infestation and he wakes up at night with bugs on him. The family is poor and in need of resources but the child’s basic needs are met. Steven doesn’t require any medical treatment. Kevin also told the reporter his grandmother yelled and screamed at him the last time the school called her about the bed bugs.
The mother was seen in the office today by me again. It was a result of my calling her up yesterday that she showed up. Her dress was very simple, not what I espected at all. She seem ambivalent about coming to see me – didn’t know what I was going to do I guess. Most of the time we talked about Charlie.
Julie and her sister (Kay) met at the Woodman office for the DD Waiver Screening. Due to the declining health of their Julie and Kay parents, Julie is now living with Kay and her niece. Also, Kay’s and Julie's 23 year-old niece who is battling stage 4 cancer lives in the home. Kay is looking into nursing facilities for her parents and sister once her parents are in better health. Julie lived with her parents in Lake Gaston also her mother stayed home Julie had more freedom also everyone on the community knew her and she swam and freely walked around.
The most appropriate observation in this case to determine the aggressiveness of Johnny’s behavior to seek attention is event recording. Event recording is one of many ways to measure and keep track of the number of times a certain behavior happens within an allotted time frame. According to the University of Kansas (n.d.), “An observer using event recording makes a tally mark or documents in some way each time a student engages in a target behavior. The observer also records the time period in which the behavior is being observed (University of Kansas, n.d.).”
Event recording would be the best method for monitoring the child’s behavior over a long period of time. Both partial interval recording and duration recording involve the amount of time the students behavior is occurring. However, his target behavior, yelling out when not called upon, is not a time ordinated behavior, rather a quantitating behavior (or the amount of times the behavior occurs). Partial interval record also often overestimates the about of time the behavior is observed and there can be room for error while observing using duration recording (not writing down the exact time the student starts or finishes). Event recording, however, is more accurate and reliable.
