On 7-31-2015 Officers were dispatched to a disturbance at 6884 N.Ridge Rd. Upon arrival, Officer McClure and I Officer Butram spoke with Maranda Degennaro. Maranda had a strong odor of an alcoholic beverage coming from her person. Maranda’s speech was unclear and she was visibly upset. Maranda was unable to stand without assistance.
On 7/20/2015 S/O EMT Perez was dispatched to MG-601 regarding a fall. S/O EMT Perez knocked and announced his presence at the door, when there was no answer S/ O EMT Perez annonced his presnece again and began to search the apartment. S/O EMT Perez found the resident, a Mrs. Doris Cotts on her bathroom floor by her toilet. Mrs.Cotts stated that she had slipped from her motorized scooter and fell onto the floor.
After Sam’s father died in January 2013, Sam struggled to manage the property at 864 Del Rio, Encinitas, California. CCIB received an Incident Report from the LTCO Maged Mansour (213) 351-6433 regarding resident Maria Alcantara. The SOC 341 states that Maria disclosed to facility staff and her Day Program staff that residential facility staff member Gloria G hit her in her back. Once notified by of incident the facility staff notified the administrator Kendal Woods (323) 208-0133. According to the RP the incident is under investigation. The suspected abuser would be suspended pending
The reporting party (RP) stated there were a list of issues and concerns regarding the level 4 home. A resident named Tammy Sanchez was observed by Regional Center counselor staff member Constance Sifton standing on a street corner on Mooney Blvd., in front of MOR Furniture store unsupervised. The counselor from the Regional Center pulled over and spoke with the resident. The resident disclosed that she had been on the street for an extended period which was indicated by the resident being drenched in sweat and appearing dehydrated and shaking. The resident was initially with a staff person named Isabelle and her boyfriend.
On 8/2/2015 SO EMT Perez was dispatched to FC-505 regaurding general sickness. SO EMT Perez knocked and announced his presence at the door and was greeted by the residents spouse and then to the resident in question, a Mrs. Barbra Myers. Mr. Myers stated that Mrs. Myers was feeling unwell for the past couple of days but today she was acting unusal and slurring/mubling her words and all around acting uncoordinated. Mrs. Myers seems to be somewhat disorientated but is answering all questions appropriatly. SO EMT Perez performed and assesment which revealed the following; Blood Pressure 148/72, Pulse 101, Spo2 96%, Mrs. Myers is very hot to the touch but does not appear to be perspiring, SO EMT Perez recieved a temperature reading from Mr. Myers '
On 8/1/2015 S/O EMT Perez was dispatched to FC-609 regarding a fall. S/O EMT Perez announced hispresnece and knocked at the door and was verbally greeted in by the resident. The fallen resident, a Mrs. Ida Looney stated she lost her balance and fell while trying to get up from the living room couch and was unable to get up on her own strength, Mrs. Looney was on the couch by the time S/O EMT Perez arrived. Mrs. Looney stated that; she was unaware of any change in medications nor was she aware that she was on any blood thinners, She did recall and remember the fall and was unaware and or could not recolect weather or not she was seeing a doctor here at riderwood. Mrs. Looney seemed to be a bit complacent mentally to which her spouse said was
On 03/07/2016 Ms. Rodriguez cam e in to the SSO to meet with HS De La Torre in order to develop a Housing Plan. HS and Ms. Rodriguez discuss about the following during the meeting. Client arrived on time well dressed and groomed. Ms. Rodriguez informed HS that family was evicted from the following address 41 Whipple St. Brooklyn, NY. Family resided at this location from 2007 until 2014.
Ms. Kathleen Ransom completed an anger management intake with Journey Counseling. Journey Counseling is a certified agency to provide anger management, domestic violence, and drug and alcohol treatment. Furthermore, I am a Colorado certified addictions counselor, senior level, and hold a Masters in social work. I am also a state approved domestic violence treatment provider.
Patient is a 51 year old Caucasian female living alone in her own home. The patient has a history and current syntoms of geralized anxity disorder, social phoina and panic attacks, which she takes madications for and sees a psychrist 2-3 times per month in Havasu. Patinets lives with six cats and the house environment is somewhat cluttered. The patinet is orinally from Glendale, CA., has a brother in Palmdale, CA, who is stays in contact with sometimes. Pt has a daughter in Big River, who is not supportive of her, but has three grandchildren who visit her.
Ms. Alexandra Estrada is a 45 years old Hispanic female. She arrived at BHWS from HELP USA on 11/20/2013. She is currently living in the Department of Homeless Services System since 12/25/2012. Ms. Estrada stands 4”10’ inches tall and weighs approximately 100 lbs. She is of olive skinned complexion, has bleached dyed blond short hair, and has distinct scars on her forehead and left arm.
The wellness of a human being has been broken down into five sections. These sections are: emotional wellness, intellectual wellness, physical wellness, social wellness and spiritual wellness. I think my strongest section for my personal wellness is my emotion, intellect and physical wellness. However, my weakest sections are social wellness and spiritual wellness. I believe that my emotional wellness is my strongest section.
As the assessment begins for Mrs. Adams there are many things that need to be addressed. Educating the patient on active participation in her healing will be of extreme importance. The interventions are only as good as the patient’s willingness to comply. We would need to begin with gaining control of the cleanliness of the apartment and that of Mrs. Adams. Opening the windows and allowing the sunlight to enter is important along with attempts at fixing the air conditioner.
