Great post and Thank you for sharing your AACN Self-Assessment score. I totally agreed with you “Nurses must be active participants in the design of automated documentation to ensure that information is recorded appropriately and in a format that can be accessed and useful to all healthcare providers” (Hebda & Czar 2013). Nursing has evolved into a world of electronic records. It is no longer only important that your nursing skills be excellent, but your ability to use a computer to document, give medicines, send specimens, check patients’ status, submit orders, and access data. “The tools and technology that nurses need to perform their work efficiently are changing in such a way that informatics competencies play a greater role” (Hebda &
While they both desire to reduce the stigma associated with mental illness, their respective objectives for addressing Canada's mental health crisis are different. The objectives of CAMIMH are to; establish a nationwide voice for mental health in Canada, to engage Canadians in conversations about mental health to decrease stigma, and to implement policies in Canada to ensure that those who suffer from mental illnesses can receive timely, appropriate care. They represent 16 mental health groups who work to advocate for health care providers, patients with mental illness, and their families. CAMIMH has been working along side the government to assist in boosting the budget for mental health on an annual basis in order to fulfill its goals and pledges. Additionally, they focus on formulating reports on the actions the government is taking and engage in media discussions of the issue and their initiatives.
In the excerpt from “the Unauthorized Autobiography of me”, Sherman Alexie conveys to the reader his nostalgia for when he was a child. He also attempts to emphasize that since that time, his life has become more lonely, and that he doesn't have “protection against the cold”. Alexie accomplishes this goal by discussing a happy moment presumably from his childhood, and he then uses an extended metaphor to explain how he now no longer has these types of connections with the people around him. The first line of the excerpt, “ my cousins are good warriors” is a reference to the fact that when he was growing up on the reservation, western movies taught him that Native Americans were fierce warriors. Although this sentence does not tie in with much of the rest of this excerpt, it is important to other parts of the work discussing conceptions of Native Americans.
Psychosocial Assessment: Summary When preparing to complete this assignment I had several road blocks. The first one was figuring out how to format the assessment, and what assessment style to use. St. Francis has a couple of assessments that they use, but they require training that I have not use completed. St. Francis is also in the process of update the assessments, and even pondering the idea of bringing in new ones. After I had completed this assignment, my supervisor told me that I would be one of the adoption workers who would kind of be the genuine pig for the new assessments starting in May. The other barrier that I would have to go through is that of confidentiality.
Yet another is that even by the definitions offered it is obvious that there is ‘Mental Health’ and there is and there is ‘Mental Illness,’ however, there is no clear definition of ‘Good Mental Health,’ being the opposite end of the spectrum. The Canadian Mental Health Association has said the following on their Toronto-based website: ‘Definitions of mental health are changing. It used to be that a person was considered to have good mental health simply if they showed no signs or symptoms of a mental illness. But in recent years, there has been a shift towards a more holistic approach to mental health. (CMHA Toronto Website,2015.)

• These classifications do not account for people who have ‘atypical’ symptoms or those that do not clearly follow the ‘script. • Critics claim that categorical systems reduce humans to one-dimensional sources of data rather than encouraging practitioners to treat the whole person. • Critics also see these systems as tools for social control – giving mental health professionals control over people’s lives. 3. Explain two alternative frameworks for understanding mental distress.
Self assessment is a concept familiar to me however one that I am not very comfortable with for it further clarifies my realization that I have so much to learn in every aspect of my life. It does keep me grounded and able to learn from anything and everything on a daily basis. I work in the Neonatal Intensive Care Unit and as a Neonatologist I am the team leader in a multitude of situations which require quick, precise and accurate decision making skills on a daily basis. These decisions are not always huge life altering moments but those that can have an impact on a baby and a family.
Whether you are performing a comprehensive / general survey assessment or a focused assessment, there are four basic tools employed which includes inspection, auscultation, percussion, and palpation (Jarvis, 2008). These methods recommended to be utilized in a well arranged manner from least disturbing or invasive to most invasive to the patient (Jarvis, 2008). Inspection is the first and most commonly used and during this assessment, the provider is striving to identify conditions that can be seen with eyes, ears, or nose such as skin color, bruises or rash, size of body parts, hair, ear, eyes and abnormal findings, sounds, and smell etc (Javier, 2015). For instance, Baid Heather 2006, suggested that if a patient walked into the room or facility, gait maybe observed, if on wheelchair or bed rest, posture issue will be noted while listening to the patient tone of voice or breathing will indicate neurological issues such as dizziness, inability speaking, difficulty swallowing, headache, head injury , vision issue, discharge from ear etc and respiratory issues hence this assessment is vital as it can lead to more evaluation and findings (Jarvis, 2015). Another is Auscultation which succeeds inspection, especially with abdominal assessment which is required to be auscultated prior percussion or palpation to curb production of false bowel sound because the assessment requires quiet to prevent false sounds and done over bare skin, listening to one sound at a time
Self-Evaluation on Counselling Session (One) Session start with a warmth greeting, by asking client ‘How can I address you?”, to be honest I felt nervous at the beginning of the session, too focus or particular on the name of client, client actually felt uncomfortable. Thereupon, I briefly explained the length of time of the session, confidentiality and the limitation. I believe that I deliver a clear message to the client.
