I responded to 106 Wildridge Road in reference to a civil complaint. Upon arrival, I spoke with Mark and Anna Sanders. According to the report Jonathan 's diagnosed with Oppositional Defiant Disorder (ODD) and other undiagnosed developmental disorders. The report revealed that both residents engaged in the incident mutual and consensually. Resident Mullin is diagnosed with bipolar disorder and intermittent explosive disorder as well as pervasive developmental disorder. According to the report both residents have been separated however they continue to reside in the group
CCIB received an Incident Report from the LTCO Maged Mansour (213) 351-6433 regarding resident Maria Alcantara. The SOC 341 states that Maria disclosed to facility staff and her Day Program staff that residential facility staff member Gloria G hit her in her back. Once notified by of incident the facility staff notified the administrator Kendal Woods (323) 208-0133. According to the RP the incident is under investigation. CCIB LPA Perryman-French received a call from Mildren. her husband Julian lives in this facility. He is non-ambulatory and requires assistance to utilize the bathroom. Mildred stated that when staff call in, the director does not replace them or cover behind them, the result is that her husband cannot get the assistance he needs to use the bathroom. This has increased his accidents.

On 8/5/15 worker visited the residence of Ms. Mona Watts, for the purpose of assessing her need for services and making first victim contact. Upon entering, the home worker noticed what appeared to be sewage under the trailer beside the steps. The home smelt like the powder off a dog flea and tick collar. There as a dog in the home and according to Ms. Watts, the dog is elderly and unable to use the bathroom outside and that was the reason there was dog urine and feces in the floor. the dog did not appear to be an elderly dog.
The reporting party (RP) resident Lucia Fields age 35 DOB: 6/25/81, stated she was admitted to the facility on 3/17/16 due to a vehicle accident which left her with 2 fractures in her back and 4 fractures in her pelvis. In the U.s, upwards of ten million pets suffer from animal abuse. Animal abuse usually goes unnoticed but is an extremely serious issue that leads to other forms of abuse and violent crimes. Animal abuse often leads to the animal being wounded or even dying. It is also a precursor to many other forms of abuse and more serious crimes.
