A. The needs assessment shows the population that should be targeted is Hispanic females between the ages of 20-59 who live in rural areas of the in Miami- Dade County. According to the Corazon y Alma profile, the hospital has a high C-section rate and many of these patients were considered high risk. The hospital has enhanced OB services but access to prenatal care is absent and this has caused a gap in care. There are financial, geographic and social barriers in the community which contribute to the gap. Access to high quality health care is a key component in supporting a healthy community and one of the reasons why Corazon y Alma Clinic is instrumental to the rural community.

The new service line of prenatal care will impact nearly every area of Miller’s Impact Assessment Framework. 1. Individual- Preventative care will be available to the patient. They will have access to information and tools that will affect their care during pregnancy. However, we don’t know if that will affect their behavior regarding appointments and no-shows. 2. Health Services- Negotiate with payors for compensation structures that align financial incentives. Standardized care process and protocols are important and will be necessary. Integrate non-health services, such as transportation, with the community. 3. Community- Those individuals which qualify for Medicaid will receive assistance when enrolling. Outpatient areas require more interpreters on site. Creation of the prenatal service line is to predict, identify, and proactively treat the population of Miami- Dade County.
