According to R.T. Dan L. Hobbs in his article in the Radiologic Technology Magazine, “the chest radiograph is the most common radiographic procedure performed in medical imaging departments, and one of the most often repeated exams.” The Fall and Winter months bring about, not only a change in our weather, but also bring about an increase in clinical visits from individuals afflicted with a variety of seasonal illnesses such as; influenza, upper respiratory infections, and pneumonia. It is the job of the radiographer to ensure that the proper exams, as well as techniques are used to give the attending physician an internal view of the lungs, heart and thoracic viscera. After verifying the patients information, as well as possible pregnancy for females, it is important that there are no artifacts, such as jewelry or buttons, anywhere near the chest area, as they may appear on the images that are to be taken. It is important to explain the procedure that is to be performed to patient so they know what to expect and to make sure they are able to cooperate with positioning or will need assistance. The two routine chest projections that are taken by the radiographer are the posterioranterior (PA) projection as well as the lateral projection of the chest. These projections are preferred in the upright position so that any air and fluid levels can be clearly seen in the images taken, but they can also be obtained in the supine position if the patient is unable to stand, due to immobility. A 72” SID is also the distance used for this projection to minimize magnification of the heart and to get a clear image of both lungs. One more important factor to obtain the best image of the lungs possible is to have the patient roll their shoulders forward to move the scapulae posteriorly and out of the way so the lungs are clearly scene.
