Improvement Plan of program strategies responsive evaluation The AAHP is working hard to reach out to the African Americans, African, Caribbean Americans communities through the various activities in these communities based on the core competencies of public health. In my improvement plan, I will use communication skills, cultural competency skills, and community dimensions of practice skills with goals and objectives to improve the understanding of the participation of this population to actively participate in the activities to improve their health. When developing public health programs, it is important to identify the target population, public health issues, and intervention for the population in the community that is going to be served. This will help to communicate orally and in writing by using the appropriate culturally proficient and age appropriate material with images to the population during health fairs and classes. Communication skills will help me to request for assistance and input from staffs, individuals and community-based and governmental organizations and other organizations for the community’s health improvement. It will also help to promote social media communication, formative conversation, town hall meetings, newsletters, radios and mobile phone apps communications. I will apply cultural competency to describe concepts of diversity pertaining to the individuals’ cultural values, languages, socioeconomic status, health issues, religious affiliations, and historical experiences. I will have high preventive solutions that can be provided by the organization and mobilize the community and local action and coordinate with stakeholders in the local community to reduce the health issues affecting African Americans population. My goal and objective are to increase the percentage of understanding and participation in the African American Health program by reaching out to the people in the community with the information to build trust which will then help them to get into informal education programs. This will be done by specific, measurable, achievable, results-focused, and time-bound (SMART) goals. By December 20, 2018, an increase of 20% improvement in one-on-one time session with each individual and families through home visits or phone calls to monitor and help them improve their lifestyles. By December 2017, an increase of 80% distribution of brochures and with door to door interactions with the individuals in the community. By September 2018, increase 70% of people to engage in community health fair and diabetes and cardiovascular classes that support and allow interactions with teaching and helpful handouts.
