Conceptualisation is a process of forming ideas which are utilized for development of a research design. It helps to know the actual data need to be collected and give to direction to an entire research process. This study model has been divided in to three phases as input, throughput, output and feedback. The grading system consists of 5, the 1-intact skin(no ulcer in a high risk foot), 2- superficial ulcer of the skin or subcutaneous tissue, 3- ulcers extend into tendon, bone, or capsule, 4- gangrene of toes or forefoot and 5- mid foot or hind gangrene. The Bates Jensen wound assessment scale also used in this study. In conventional treatment group and in herbal treatment group the stage, grade and bates Jensen wound assessment tools were assessed in pre-test and post test 1 after 15 days and post test 2 after 30 days. In the conventional group the wound dressing was done by using of povidonee iodine ointment and in herbal treatment group the wound dressing was done by using the new herbal formulation which was prepared by the researcher. Herbal oil is prepared by using 50 grams of turmeric (Curcuma longa) powder dissolved in 1000 ml of coconut oil (Cocos nucifera) with 1000 gm of green neem- (Azadirachta Indica) leaves boiled up with, 2040C(4000F).
