On this date worker made an unannounced visit to the home of Mrs. Molly Tigger, for the purpose of assessing her need for services. Worker knocked on the door and Ms. Tigger yelled come on in. Worker entered the home and followed the sound of Ms. Tigger's voice. Ms. Tigger was lying in a hospital bed located in a bedroom. The home was clean and free from any foul odors. Ms. Tigger was appropriately dressed with good personal hygiene. Worker introduced herself and explained the reason for the visit. Ms. Tigger informed worker she had not been a Tigger in years her current last name was Westbrook and she instructed worker to call her Ms. Molly. She lives with her granddaughter Toni who is her caregiver but Toni is not home much. According to Ms. Molly Toni left the home around 12-1pm and would not return until around 7pm. Ms. Tigger is bedbound but can transfer herself from the bed to the bedside commode and wheelchair. At the beginning of the interview Ms. Tigger stated she was hungry. In the kitchen, there was very little food bologna and cheese but no bread. The refrigerator consisted mostly of takeout food and a subway sandwich on the kitchen table. When questioned about food she stated she needed more food stamps. She provided worker with some mail that informed her she was getting a decrease in her food stamps 30.00 to 28.00 each month.
