Every Health and Social worker has this process or 'cycle' to ensure that every stage of the care panning process is covered. There are five stages to this process 1. Assessing needs- Needs assessment are for people who may need help because of a disability, ill health or old age. 2. Planning- The process of making plans for the reasons service provision. 3. Implementing- Putting the plan in to effect. 4. Monitoring- Observing and checking the progress of the plan over a period of time. 5. Review/Evaluation- Reviewing or evaluating the service provision what's working what isn't, or if its still positively working
