Juanita works part-time providing support, assistance, and training to four residents who reside in Shurm Heights group home. Juanita has built a good rapport with the residents. Juanita keeps her supervisor informed of any problem areas or health/safety concerns. Juanita provides supports the residents with all aspects of daily living such as grooming, laundry, personal hygiene, medication compliance, and household maintenance. She utilizes person-centered practices when working with the residents. Juanita supports the residents with social, recreational outings and money management. Juanita expands her knowledge of individuals with disabilities through in-services, and annual trainings. Juanita’s TOVA, First Aid, CPR, Human Right, Code of Ethics, Confidentiality, Fraud Policy, Prevention of Infectious Disease, Cultural Competency, HIPPA Regulations, Incident Reporting, and Defensive Driving are current and up to date. …show more content…

Also, Juanita had frequent last minute call-outs for her shifts without securing coverage (8/21, 9/28, 1/8, 1/24, 3/7, and 4/29). This lack of coverage has caused hardships for other employees in the Shurm group home. This hardship was recently evident during the 1/24/16 snow event and Juanita failed to report to work or secure coverage. She is an “Essential Employee” for HAMHDS and required to work during inclement weather and other emergency events.

Goal: Juanita will initiate, plan, and coordinate opportunities for community outings with the residents, check e-mail once each shift, ensure that documentation is completed accurately; including her timecard, proof read documentation to ensure that outcomes are addressed accordingly and participate in one staff meeting per
